Zusammenfassung
Bei 24 Patienten wurde der Ductus thoracicus im Bereich des Angulus veno-lymphaticus freigelegt. Vier Patienten hatten davon eine intakte Leber. Kaliber und Lymphfluß des Ductus thoracicus entsprachen hier den schon bekannten Normalwerten. Bei den 20 Patienten mit Lebercirrhose war der Lymphfluß in 18 Fällen erhöht, eine Erweiterung des Ductus thoracicus fand sich aber nur in der Hälfte der Fälle. Die Kanülierung des Ductus thoracicus mit äußerer Lymphdrainage haben wir ohne Erfolg bei zwei Patienten mit Ösophagusvaricenblutung und mit wechselndem Erfolg bei sechs Patienten mit massivem Ascites durchgeführt. Als therapeutische Maßnahme wird die Kanülierung des Ductus thoracicus beim Ascites wegen ihrer kurzdauernden Wirkung und wegen der nicht ungefährlichen Komplikationsmöglichkeiten von uns abgelehnt. Die cervicale veno-lymphatische Anastomose vermeidet die Nachteile der äußeren Lymphdrainage, ihre Wirkungsweise ist jedoch keineswegs geklärt. Wir haben sie bisher bei sechs Patienten ausgeführt, davon in zwei Fällen ohne Erfolg wegen Ösophagusvaricenblutung, und in vier Fällen wegen eines massiven Ascites. In zwei Fällen kam es dabei zu einer nun schon 1/2 bzw. 1 Jahr anhaltenden vollständigen Rückbildung des Ascites.
