Tumor of the glomus jugulare co-existing possibly with carotid body tumor

Summary and conclusions

The authors presented a case of tumor of the glomus jugulare, co-existing with a newgrowth of the antero-lateral region of the neck, which resembled clinically a carotid body tumor. Whereas a histological examination disclosed the exact nature of the tumor in the middle ear, the character of the cervical neoplasm could only be assumed.

After having described the symptomatology and the evolution of the tumor of the glomus jugulare, a review of the literature is made, insisting upon the small number of publications concerning this subject, and upon the neurological syndrome, which characterises the basal infiltration of these tumors.

The surgical treatment is discussed, considering that total removal of glomus jugulare tumors is almost impossible at a stage when the base of the skull is already infiltrated.

Die Verff. berichten über einen Fall einer Geschwulst des Glomus jugulare, die von einer Neoplasie der vorderseitlichen Halsgegend begleitet und klinisch mit einem Tumor des Glomus carotideus zu vergleichen ist. Während die histologische Untersuchung die genaue Tumorart im Mittelohr offenbarte, konnte der Charakter der cervikalen Geschwulst nur vermutet werden. Nach Beschreibung der Symptomatologie und Entwicklung der Geschwulst des Glomus jugulare erörtern die Verff. die in der Literatur erwähnten Fälle und weisen auf die geringe Anzahl von Veröffentlichungen auf diesem Gebiet und auf das neurologische Syndrom hin, das die basale Infiltration dieser Tumore kennzeichnet.

Die Verff. besprechen die chirurgische Behandlung und meinen, daß die totale Exstirpation der Geschwulst des Glomus jugulare fast unmöglich sei wenn die Schädelbasis bereits infiltriert ist.

Résumé

Les auteurs présentent l'observation d'un cas de tumeur du glomus jugulaire, coexistant avec une néoformation de la région antéro-latérale du cou qui présentait les caractéristiques cliniques d'une tumeur du corpuscule carotidien. L'examen histologique de la tumeur située dans l'oreille moyenne a permis son identification, alors que la nature histologique de la tumeur du cou reste hypothétique.

Après avoir décrit la symptomatologie et l'évolution des tumeurs du glomus jugulaire, les auteurs passent en révue la littérature du sujet, en insistant sur le nombre réduit de travaux et sur le syndrome neurologique caractérisé par la tendance qu'ont ces tumeurs d'infiltrer la base du crâne.

Le traitement chirurgical des tumeurs du glomus jugulaire a été discuté, les auteurs considérant l'extirpation complète des tumeurs du glomus jugulaire comme étant pratiquement impossible dans une période de l'évolution où il existe déjà une infiltration de la base du crâne.

Gli AA riferiscono su di un caso di tumore del glomo giugulare coesistente con una neoplasia della regione antero-laterale del collo, paragonabile clinicamente ad un tumore del glomo carotideo. Mentre l'esame istologico permetteva di desumere la natura esatta del tumore nell'orecchio medio, il carattere della neoplasia cervicale poteva essere soltanto sospettato.

Avendo descritta la sintomatologia e l'evoluzione del tumore del glomo giugulare, gli AA fanno una revisione dei casi della letteratura, tenendo presente lo scarso numero di pubblicazioni su questo argomento e sulla sindrome neurologica che caratterizza l'infiltrazione basale di questi tumori.

Discusso il trattamento chirurgico, gli AA concludono che l'asportazione totale del tumore del glomo giugulare è quasi impossibile, quando la base cranica è già stata infiltrata.

Resumen

Los autores describen el caso de un tumor del glomus yugulare coexistente con una neoplasia en región anterolateral del cuello, cuyos caractères clínicos eran superponibles a los de un tumor del glomus caroticus. El análisis histológico reveló la naturaleza exacta del tumor en el oído medio, pero en cambio solo pudo sospecharse el caracter de la neoplasia cervical.

Después de describir la sintomatología y desarrollo de los tumores del glomus yugulare, hacen los autores una revisión de los casos de la literatura y señalan el escaso número de publicaciones. Asimismo el síndrome neurológico de estos tumores, que indica una infiltración de la base.

Discuten el tratamiento quirúrgico y llegan a la conclusión de que la extirpación total de un tumor del glomus yugulare es casi imposible, cuando la base del cráneo esta ya invadida.
