Zusammenfassung
Nach Besprechung der dem Studium der Gefäßmißbildungen innerhalb des Wirbelkanals eigenen Schwierigkeiten, berichten die Autoren ihre 14 Beobachtungen, das sind 0,75% der Krankheitsfälle, die wegen Wirbelsäulen- oder Rückenmarkserkrankungen im Zeitraum von 1940 bis 1960 imNeurochirurgischen Institut in Santiago de Chile zur Aufnahme kamen.
Die Autoren teilen diese Gefäßmißbildungen in epidurale und subdurale ein. Sie unterteilen die epiduralen in 2 Gruppen:

Gruppe I
(5 Fälle): Epidurale, racemöse, venöse Angiome. Sie verursachen ein im allgemeinen polyradikuläres Schmerzsyndrom der unteren Kreuzbeingegend, das durch den Wechsel des venösen Druckes beeinflußt wird und führen manchmal zu lokalen Knochenerosionen eines Wirbels. Es wird der Einfluß der Schwangerschaft auf die Symptomatologie dieser Miß-bildungen besprochen.

Gruppe II
(4 Fälle): Epidurale, arteriovenöse Aneurysmen und epidurale, kapilläre Angiome. Sie verursachen das typische Bild einer Rückenmarks-kompression mit oder ohne Wirbelbeteiligung. Die subduralen Mißbildungen werden ebenfalls in 2 Gruppen unterteilt:

Gruppe III
(4 Fälle): Venöse, racemöse und arteriovenöse, racemöse, medulläre Angiome. Sie werden gemeinsam abgehandelt, da es keine wesentlichen klinischen Unterschiede zwischen diesen beiden Abarten gibt. Man betont ihre progressive Tendenz trotz der Entlastungslaminektomie.

Gruppe IV
(1 Fall): Ponto-bulbo-medulläre teleangiektatische Angiomatose. Man berichtet über einen Fall mit multiplen teleangiektatischen Herden in der Brücke, im verlängerten und im Rückenmark, der aber klinisch nur medulläre Symptome erkennen ließ. Die Autoren glauben, daß dieser Fall, ebenso wie die Fälle der III. Gruppe eine Variation der angio-dysgenetischen Myelopathien darstellt, ähnlich der Foix-Alajouanineschen Krankheit.
Die Autoren halten die Ergebnisse der chirurgischen Behandlung in den Gruppen I und II für ausgezeichnet und betonen die Gefahren eines direkten Eingriffs und den geringen Erfolg der Entlastungslaminektomie und Röntgentherapie in der Gruppe III.
Was die Fälle der Gruppe IV betrifft, so halten sie eine klinische Diagnose für unmöglich.
Summary
After a survey of the difficulties concerning the study of spinal cord vascular malformations the authors report 14 cases, that is 0.75% of all the patients interned at theInstitute of Neurosurgery of Santiago de Chile during the period 1940 to 1960 for spinal cord pathology.
They divide the malformations in extradural and subdural groups. The extradural are classed as follows:

Group I
(5 cases): Racemous venous epidural angiomas. They are characterized by a painful sindrome, usually polyradicular in the lower lumbar region, which is modified by changes in venous pressure and ocassionally produces an erosion of a vertebra. Its relation with pregnancy is seen.

Group II
(4 cases): Arteriovenous epidural aneurysms and epidural capillary angiomas. They give a typical picture of spinal cord compression with or without vertebral lesion.
The subdural cases are classified thus:

Group III
(4 cases): Racemous venous angiomas and racemous arteriovenous angiomas of the spinal cord. They are analyzed together since there are no clinical differences between them. Emphasis is given to their progressive course in spite of decompressive laminectomy.

Group IV
(1 case): Telangectatic angiomatosis of the pons, medulla and cord. One case with several telangectatic foci in the pons, medulla and cord giving symptoms only in the cord. This case together with group III are considered to be varieties of angiodysgenetic myelopathies similar to Foix and Alajouanine syndrome.
The authors consider surgical treatment excellent in groups I and II. They point out the dangers of a direct approach to the malformations in group III as well as the futility of decompressive laminectomy and radiotherapy.
They find clinical diagnosis impossible in group IV.

Après avoir analysé les difficultés inhérentes à l'étude des malformations vasculaires intrarachidiennes, les auteurs présentent leurs 14 observations qui correspondent au 0,75% de tous les patients avec une pathologie rachidienne et ou médullaire, internés àl'Institut de Neurochirurgie du Chile depuis 1940 jusqu'à 1960.
Elles sont séparées en malformations vasculaires situées dans l'espace épidural et celles de localisation subdurale. Ils classifient les épidurales en deux groupes:

Groupe I
(5 cas): Angiomes racémeux véneux épidurales. Ils se caractérisent par un tableau douloureux, généralement polyradiculaire, lombaire bas, qui se modifie avec les variations de la pression veineuse et produisent parfois l'érosion localisée d'une vertèbre. Ils analysent sa relation avec la grossesse.

Groupe II
(4 cas): Anévrysmes artérioveineux épidurales et angiomes capillaires épidurales. Ils produisent un tableau typique de compression médullaire, avec ou sans lésion de la vertèbre adyacente. Ils divisent les subdurales en deux autres groupes:

Groupe III
(4 cas): Angiomes racémeux veineux médullaires et angiomes (anévrysmes) artérioveineux racémeux médullaires. Comme il n'y a pas de différences cliniques importantes entre les 2, elles sont analysées ensemble. Son caractère progressif est mis en relief, malgré la laminectomie décompressive.

Groupe IV
(1 cas): Angiomatose télangectasique ponto-bulbo-médullaire: Un cas avec de multiples foyers télangectasiques en protubérance, bulbe et moëlle, qui n'ont donné que des symptômes médullaires. On conclue que ce cas ainsi que ceux du groupe III correspondent à des variétés de myélopathies angiodisgénétiques, semblables à la maladie de Foix et Alajouanine.
Les auteurs considèrent excellents les résultats du traitement chirurgical dans les groupes I et II. Ils mettent en relief des dangers d'intervenir directement sur la malformation dans le groupe III, ainsi que le peu de résultats de la laminectomie décompressive et de la radiothérapie.
On considère impossible de faire le diagnostic clinique du dernier groupe.

Dopo di analizzare le difficoltà inerenti allo studio delle malformazioni vascolari intrarachidee, gli autori presentano 14 osservazioni che corrispondono al 0,75% di tutti i pazienti con patologia rachidea e midollare ricoverati nell'Isrtituto di Neurochirurgia del Cile dal 1940 al 1960.
Le dividono in malformazioni vascolari localizzate nello spazio epidurale e quelle di localizzazione sottodurale. Classificano le epidurali in due gruppi:

Gruppo I
(5 osservazioni): Angiomi raccimosi venosi epidurali. Si caratterizzano per un quadro doloroso, generalmente poliradicollare, lombare basso che si modifica con le variazioni della pressione venosa a che a volte producono erosione localizzata di una vertebra. Si analizzano le sue relazioni con la gravidanza.

Gruppo II
(4 osservazioni): Aneurismi arterio venosi epidurali e angiomi capillari epidurali. Producono un quadro tipico di compressione midollare, con o senza alterazione della vertebra adiacente.
Le malformazioni di ubicazione sottodurale sono divise in altri due gruppi:

Gruppo III
(4 osservazioni): Angiomi raccimosi venosi midollari e angiomi (aneurismi) arterio venosi raccimosi midollari che si analizzano nello stesso gruppo perché impossibile differenziarli clinicamente. Si fa notare il suo carattere progressivo malgrado la laminectomia scompressiva.

Gruppo IV
(1 osservazione): Angiomatosi telangiettasica ponto-bulbo-midollare: una osservazione a localizzazione molteplice ma di sintomatología esclusivamente midollare. Concludono que questa osservazione assieme a quelle del gruppo III corrispondono a variazioni di mielopatie angiodisgenetiche sommiglianti al sindrome di Foix e Alajouanine.
Gli autori considerano eccellenti i risultati del trattamento chirugico nei gruppi I e II e fanno notare i pericoli dell'intervenzione nella malformazione del gruppo III come anche lo scarso rendimento della laminectomia scompressiva e della radioterapia. Considerano impossibile la diagnosi clinica dell'ultimo gruppo.

Después de analizar las dificultades inherentes al estudio de las malformaciones vasculares intrarraquídeas, los autores presentan sus 14 observaciones, que corresponden al 0,75% de todos los pacientes con patología raquídea y medular hospitalizados en elInstituto de Neurocirurgia de Chile desde 1940 a 1960.
Las dividen en malformaciones vasculares ubicadas en el espacio epidural y las de localizatión subdural. Clasifican las epidurales en dos grupos:

Grupo I
(5 casos): Angiomas racemosos venosos epidurales. Se caracterizan por un cuadro doloroso, generalmente polirradicular, lumbar bajo, que se modifica con las variaciones de la presión venosa y a veces producen erosión localizada de una vértebra. Se analiza su relación con el embarazo.

Grupo II
(4 casos): Aneurismas arteriovenosos epidurales y angiomas capilares epidurales. Producen un cuadro típico de compressión medular, con o sin lesión de la vértebra adyacente. Dividen las subdurales en otros dos grupos:

Grupo III
(4 casos): Angiomas racemosos venosos medulares y angiomas (aneurismas) arteriovenosos racemosos medulares. Se los analiza en conjunto, ya que no hay diferencias clínicas de importancia entre ambos. Se destaca su carácter progresivo, a pesar de la laminectomía descompresiva.

Grupo IV
(1 caso): Angiomatosis telangectásica ponto-bulbo-medular: Un caso de múltiples focos telangectásicos en protuberancia, bulbo y médula, que sólo dieron síntomas medulares. Se concluye que este caso, así como los del Grupo III, corresponden a variedades de mielopatías angiodisgenéticas, semejantes al cuadro de Foix y Alajouanine.
Los autores consideran excelentes los resultados del tratamiento quirúrgico en los grupos I y II y destacan los peligros de intervenir directamente sobre la malformatión en el grupo III, así como el escaso rendimiento de la laminectomía descompresiva y de la radioterapia.
El último grupo lo consideran imposible de diagnosticar clínicamente.
