Zusammenfassung
Nach einer subokzipitalen Myelographie mit Pantopaque wegen eines rezidivierenden RM-Tumors kam es dabei in engem zeitlichen Zusammenhang zur Erblindung einer 31jährigen Patientin. Neben einer „Neuromyelitis optica“ mußte ein „Kontrastmittelschaden“ diskutiert werden. Die Sektion enthüllte ein relativ kleines medulloblastisches Gewächs im Velum medullare post. und im Kleinhirnunterwurmgebiet, das zu einer ausgedehnten Metastasierung im Ventrikelbereich, einer völligen geschwulstigen Ummauerung des Rückenmarks von D 4 bis zum Konus sowie zur Infiltration der Basalzisternen und der Optikuskreuzung führte. Daneben konnte eine Kontrastmittel-Imbibition der Basalzisternen makroskopisch und histologisch nachgewiesen werden. Eine indirekte Mitwirkung des Kontrastmittels an der Erblindung der Patientin wird wegen desengen zeitlichen Zusammenhangs mit der subokzipitalen Myelographie angenommen. Die Differentialdiagnose zur Neuromyelitis optica wird erörtert. Schließlich wird die Herkunft der medulloblastischen Gewächse aus der „Kopfneuralleiste“ besprochen und näher begründet.
Summary
In a 31 year old female patient suboccipital myelography with pantopaque carried out on account of a recurring RM-tumour coincided in a close time relationship with loss of vision. In addition to a “neuromyelitis optica” the possibility of contrast medium damage must be discussed. Autopsy revealed a relatively small medulloblastic growth in the posterior medullary velum and in the region of the lower vermis, which had led to pronounced metastases throughout the ventricles and to a complete tumorous coating of the spinal cord from D 4 to the conus, together with infiltration of the basal cisterns and the chiasm. Microscopically and histologically, take up of contrast medium in the basal cisterns could be observed. On account of the close time relationship with the suboccipital myelogram an indirect synergy of the contrast medium leading to the patients visual failure is accepted. The differential diagnosis of neuromyelitis optica is discussed. Finally the origin of medulloblastic growths in the “cerebral neural ridge” is discussed and more precisely substantiated.
