Summary

Multiple arterio-sinusal fistulae in five patients have been reported. They were found at all sinuses, but predominantly at the transverse sinus. Meningeal and scalp branches from the external carotid and vertebral arteries, also branches from the internal carotid and even from the subclavian arteries participated in the shunt.

The main symptoms were intracranial murmur and headaches. These fistulae result from a developmental defect of the capillary system and ascending veins, and they become patent gradually in adolescence.

The only successful treatment is surgery close to the sinus and better still, of the sinus itself.

Zusammenfassung

Wir berichten über 5 Patienten mit multiplen arterio-sinuösen Fisteln, welche an allen Sinusen, hauptsächlich aber am Sinus transversus gefunden wurden. Die meningealen Äste und die Schädeläste der Arteriae carotis externae und vertebrales, wie auch der Arteriae carotis internae und sogar der Arteriae subclaviae nahmen daran teil.

Die wichtigsten Symptome waren Kopfschmerzen und intracraniales Geräusch. Die Fisteln sind die Folge einer Entwicklungsstörung des Kapillarsystems und der ascendenten Venen, welche erst später im Erwachsenenalter klinisch in Erscheinung tritt.

Die einzige erfolgreiche Therapie ist der chirurgische Eingriff in der Nähe des Sinus, am besten am Sinus selbst.

Nous informons de 5 malades avec des fistules arteriosinusales multiples. On pouvait les découvrir sur tous les sinus, mais avant tout sur le sinus transversus. Les branches méningées et craniennes des carotides externes, des artères vertébrales, des carotides internes et même des artères subclaviennes y participaient.

Les symptômes principaux étaient des maux de tête et des murmures intracraniens.

Les fistules sont la conséquence d'un développement défectueux du système capillaire et des veines ascendentes. Elles se manifestent dans l'âge adulte.

La seule action thérapeutique qui peut avoir succès est une intervention chirurgicale non loin du sinus, le mieux sur le sinus lui même.

Gli Autori riportano 5 casi di fistola multipla arterio-sinusale, trovate a livello di tutti i seni, ma in modo preponderante a livello del seno trasverso. La branca meningea e cutanea dalla carotide esterna e dalle arterie vertebrali e inoltre le diramazioni della carotide interna e delle arterie succlavie partecipano alla fistola. I sintomi principali sono soffi e cefalea.

Queste fistole sono l'effetto di un difetto di sviluppo del sistema capillare e delle vene ascendenti e che si fanno visibili gradualmente nell'adolescenza. L'unico trattamento che abbia dato risultati positivi è un intervento chirurgico nelle vicinanze del seno e ancora meglio al seno stesso.

Resumen

Se presentan cinco enfermos con fístulas de tipo arterio-sinusal múltiples. Estas fístulas fueron encintrades en contacto con todos los senos, predominantemente con el seno transverso. Participaron tambien las arterias meningeales, del cuero cabelludo, de la arteria carotis externa, arteria vertebral, tambien ramas de la arteria carotis interna y la subclavia.

Los síntomas cardinales fueron el soplo y cefalea. Estas fístulas son el resultado de los defeetos del desarollo del sistema capilar y de las venas ascendentes y aumuntan gradualmente en la adolescencia.

Los buenos resultados se pueden obtener solo quirurgicamente, cerrando las fístulas o mejor los senos mismos.
