Zusammenfassung
Arbeitsunfähigkeitsdaten von ca. 50 000 BKK-Versicherten aus fünf Unternehmen des Einzelhandels und der Metallbearbeitung wurden im Hinblick auf arbeitsbedingte Gesundheitgefahren ausgewertet. Die Versicherten wurden zunächst in 83 unterschiedliche Arbeitsplatztypen gruppiert, für die sodann eine Ermittlung des Vorkommens von 70 Belastungsarten vorgenommen wurde. Schliesslich wurden Arbeitsunfähigkeit- und Belastungsdaten zusammengeführt und diagnosenspezifische relative Arbeitsunfähigkeit-Risiken berechnet. Im Hinblick auf alle Krankheitsgruppen erwies sich ein enger Handlungsspielraum als bedeutendster Belastungsfaktor, der konsistent hoch mit Arbeitsunfähigkeit assoziiert war. Bezüglich Dorsopathien z. B. ergab sich ein signifikantes relatives Risiko von 4,7 bei Beschäftigten, deren Tätigkeit durch engen Handlungsspielraum gekennzeichnet war im Vergleich zu hierdurch unbelasteten Beschäftigten. Im Gegensatz hierzu waren die Ratenverhältnisse für Belastungen durch Arbeits- und Entscheidungsdruck in der Regel signifikant kleiner eins. Physikalische Belastungsfaktoren, wie Arbeitsschwere/Lastenheben und Vibrationen, erwiesen sich ebenfalls häufig signifikant mit der Häufigkeit von Arbeitsunfähigkeit assoziiert, wenngleich die Zusammenhänge auch weniger deutlich und konsistent waren, als bei den psychischen Belastungen.
Summary
Sickness absence data of approx. 50 000 employees were gathered from the health insurance funds of five companies from the metal processing and retail trade. According to the job characteristics the employees were grouped in 83 different job types. Each job type was accessed with respect to the occurrence of risk factors (70 items). Finally, adjusted relative risks for disease-specific sickness absence were calculated. With respect to all diseases studied “low job control” turned out to be the risk factor highest associated with sickness absence. E. g., for back disorders a relative risk of 4.7 was seen for employees whose jobs were highest characterised by “low job control” compared to employees without. In contrast, relative risks concerning “high job demands” were well below one. In general, associations between physical work bad (e. g. heavy work, vibrations) and sickness absence from various diseases were observed also, but those of psychosocial factors were more consistent and dominant.
Résumé
Les données concernant l'incapacité professionnelle de 50 000 membres des caisses d'assurance maladie de l'entreprise ont été analysées au but d'identifier des risques professionnels. Les employés travaillaient dans les secteurs de vente en détail et d'usinage des métaux. Premièrement les assurés ont été groupés en 83 types de poste de travail dont les facteurs de risque professionnelle (70 catégories) ont été estimés. Puis les données concernant l'incapacité professionnelle et les données concernant les risques ont été jointes et, en conclusion, les risques relatifs d'incapacité professionnelle ont été calculés. En ce qui concerne toutes les maladies étudiées «la capacité d'agir limitée» s'est avérée être le facteur de risque le plus fortement associé à l'incapacité professionnelle. Par example, un risque relatif de 4.7 pour les incommodités dorsales a été observé pour les employés dont le travail a été caractérisé par «capacité d'agir limitée» mis en regard des employés non-chargés de ce facteur de risques. Au contraire, les risques relatifs au sujet de l'exigence professionnelle élevée étaient bien en-dessous 1. En général, on a observé des associations entre les charges professionnelles physiques (par exemple travail pénible, vibrations) et l'incapacité professionnelle à cause de diverses maladies, mais les associations concernant les facteurs de risque psycho-sociales étaient plus distinctes et plus consistentes.
