Résumé
Les organisations de soins similaires aux HMOs américaines ont fait leur apparition en Suisse. Elles suscitent beaucoup de réactions, aussi bien dans la population générale que parmi les médecins. Contrairement à l'assurance-maladie classique, les HMOs organisent et gèrent activement les soins de leurs affiliés. Cet article retrace l'histoire déjà ancienne de ces organisations en Europe et aux Etats Unis, et discute en particulier l'évolution et la diversification récentes du concept des «soins gérés». Suit une présentation et un commentaire sur les mécanismes d'action et les effets indésirables des outils de gestion des soins à disposition aussi bien des assureurs traditionnels que des organisations de soins gérés. II apparaît que tous les soins sont gérés, que tous les outils de gestion ont des effets pervers potentiels, et que l'utilisation de certains de ces outils implique une redistribution de rôles entre les assurés, les administrateurs, et les médecins. Les auteurs suggèrent que la complexité de la gestion des soins nécessite une implication plus active des professions de la santé dans ce processus.
Zusammenfassung
Seit kurzem gibt es auch in der Schweiz Formen von Pflegeversorgungssystemen, die sich den amerikanischen Health Maintenance Organizations (HMO) annähern. Diese neue Form der Organisation der Krankenpflegeversorgung und der Krankenversicherung hat in weiten Kreisen der Ärzteschaft und der Bevölkerung heftige Reaktionen hervorgerufen. HMOs, im Gegensatz zur klassischen Krankenversicherung, organisieren und verwalten in aktiver Weise die Pflegeversorgung ihrer Versicherten. In diesem Artikel wird auf die Entstehung dieser Organisation in Amerika und in Europa zurückgeblendet. Das Konzept des “managed care” wird zudem eingehend besprochen, vor allem in bezug auf dessen neueren Entwicklungen. Weiter werden die Wirkungsmechanismen, inklusive der unerwünschten Nebenerscheinungen sowie die in den klassischen Versicherungssystemen und in den HMOs verfügbaren Verwaltungsinstrumente beschrieben. Aus diesen Abhandlungen geht hervor, dass jedes Pflegeversorgungs- und Krankenversicherungssystem verwaltet sein will und dass zudem der Gebrauch von gewissen Verwaltungsin-strumenten Situationen hervorruft, in denen die Rollenverteilung zwischen Versicherten, Verwaltern und Ärzten neu zu definieren ist. Die Autoren schlagen vor, dass sich die Gesundheitsberufe, der Komplexität der Pflegeversorgung wegen, an deren Verwaltungsprozess aktiver beteiligen sollten.
Summary
Health care organizations similar to American HMOs have recently appeared in Switzerland. They elicit many reactions, both in the general public and among the medical profession. In contrast to traditional health insurance, HMOs organize and actively manage health care delivered to their members. This paper reviews the historical background of similar organizations in Europe and in the United States, and focuses in particular on the recent evolution and fragmentation of the concept of “managed care”. Follows a discussion of the mechanisms and the sideeffects of various tools used to manage care, both in managed care settings and by traditional health insurance plans. It appears that all of health care is managed, that all management tools have potential side effects, and that use of some management tools implies a redistribution of the respective roles of plan members, administrators, and physicians. The authors suggest that the complexity of health care management requires a more active implication of the health professions in that process.
