Abstract
The aim of this study was to analyze and to describe the condition of the lateral pterygoid muscles during and after functional appliance therapy in children who had sustained condylar fractures.
The treatment group consisted of 19 patients with an average age of 13.4 years, who had sustained a unilateral fracture at an average age of 8.4 years and had been treated functionally for 6 to 8 months. Muscle condition was assessed through axial solt tissue description. The volume and density of the lateral pterygold muscles were measured in both groups and were compared on the basis of sex, age and fracture type.
74% of the patients in the follow-up group showed muscle differences of more than 10% between the 2 sides. In 2/3 of these patients the lateral pterygold of the fracture side was 13 to 69% smaller. The volumes of the contralateral pterygold muscles in the 2 patient groups differed by 10% on the basis of sex. Both in the treatment group and in the follow-up group the volume difference between the healthy and fractured side was as high as 70% depending on localization and type of fracture. Deep fractures and fractures with complete dislocation of the condyle evoked the most serious reduction in muscles volume.
Zusammenfassung
Ziel der Studie war es, nach metrischer Analyse des Kondylus und des Kollums auch die posttraumatische und-therapeutische Beschaffenheit des lateralen Pterygoidmuskels während und nach der funktionskieferorthopädischen Behandlung bei Kindern mit Gelenkfortsatzfrakturen zu untersuchen und zu beurteilen.
19 durchschnittlich 13,4 Jahre alte Patienten mit einseitiger, vor im Mittel 4,9 Jahren funktionell behandelter Gelenkfraktur wurden als Nachuntersuchungsgruppe und 20 Patienten mit einseitiger Fraktur im After von 8,4 Jahren, die seit sechs bis acht Monaten funktionell behandelt werden, als Therapiegruppe in der vorliegenden Studie berücksichtigt. Die Muskelbewertung erfolgte anhand axialer Weichteildarstelungen. Bei beiden Gruppen konnten das Volumen und die Dichte des lateralen Pterygoidmuskels gemessen und geschlechts-, alterssowie frakturtypspezifisch analysiert werden.
Bei 74% der nachuntesuchten Patienten wurden muskuläre Seitendifferenzen von mehr als 10% festgestellt. Bei fast zwei Drittel dieser Patienten war der laterale Pterygoldmuskel auf der Frakturseite zwischen 13 und 69% verkleinert. Die Volumina der kontralateralen Pterygoldmuskeln beider Patientengruppen unterschieden sich geschlechtspezifisch um 10%. Sowohl bei den Nachuntersuchten als auch in der Therapiegruppe nahm die Volumendifferenz zwischen gesunder und Fraktursite bis zu Unterschieden von 70% in Abhängigkeit von der Lokalisations und Art der Fraktur zu. Tiefe Frakturen und Frakturen mit vollständiger Luxation des Gelenkkopfes aus der Pfanne bewirkten die gravierendsten Reduktionen des Muskelvolumens.
