Zusammenfassung
In der Schweizer Armee sind Instruktoren und Festungswächter berufsbedingt dem gehörschädigenden Schiesslärm besonders ausgesetzt. Es wurde untersucht, ob die 1987 bis 1996 feststellbare Abnahme der Anzahl akuter akustischer Traumata im Militärdienst auch bei Instruktoren der Armee und Festungswächtern nachweisbar ist und ob diese Personen häufiger Hörgeräte-Träger (als Indikator für eine Gehöreinbusse) sind als der Durchschnitt der männlichen Schweizer Wohnbevölkerung. Als erstes wurden die historischen Zeitreihen der Inzidenzdichte akuter akustischer Traumata für Miliz-Angehörige der Armee sowie für Instruktoren der Armee und Festungswächter untersucht. Als zweites wurde die Prävalenzrate von Hörgeräte-Trägern bei Instruktoren der Armee und Festungswächtern mit derjenigen bei der männlichen Schweizer Wohnbevölkerung verglichen. Der zehnjährige Verlauf der Inzidenzdichte von akuten akustischen Traumata bei Instruktoren der Armee und Festungswächtern zeigte keine signifikante Veränderung im Gegensatz zu einem stark abfallenden Trend (−12% pro Jahr) bei den Miliz-Angehörigen der Armee. Die Prävalenzrate für Hörgeräte-Träger bei Instruktoren der Armee und Festungswächtem war signifikant höher (RR 3, 91 (95% Cl 3,09–4,96) als bei der männlichen Schweizer Wohnbevölkerung. Die Resultate sprechen für eine erhöhte Gehörgefährdung von Instruktoren der Armee und Festungswachtern, am ehesten verursacht durch eine hohe Impulslärm-Exposition (Berufskrankheit). Es erscheint deshalb angezeigt, Impulslärm-exponierte Personen dieser Berufsgruppen einem obligatorischen Präventionsprogramm zuzuführen.
Summary
In the Swiss army instructors and fortifications guards are exposed to firearms' noise which harms the ear. It was investigated whether the decline in the cases of acute acoustic traumas in non-professional soldiers serving in the army from 1987 to 1996 also was detectable in army instructors and fortifications guards (professional soldiers) and whether these persons were wearing hearing aids (as an indicator of hearing loss) more often than the average of the male Swiss population. First, we analyzed a historical time series of the incidence density of acute acoustic traumas in non-professional soldiers as well as in army instructors and fortifications guards. Second, we compared the prevalence rate of wearing a hearing aid in army instructors and fortifications guards with the male Swiss population. The incidence density for acute acoustic traumas in army instructors and fortifications guards observed over ten years showed no significant change whereas among non-professional soldiers a strong decline (−12% per year) was observed. The prevalence rate of wearing a hearing aid in army instructors and fortifications guards was significantly higher (RR 3.91 [95% Cl 3.09–4.96]) than in the male Swiss population. These results suggest increased hearing impairment among army instructors and fortifications guards which is probably due to the high exposure to impulse noise events (occupational disease). It is recommended that these persons be subjected to a compulsory prevention program.
Résumé
Dans l'armée suisse, les instructeurs et les gardes-fortifications sont tout particulièrement exposés aux bruits de tir qui atteignent l'ouïe. On s'est demandé si la diminution du nombre des traumatismes acoustigues pendant le service chez les militaires durant la période 1987 jusqu'à 1996 était aussi visible chez les instructeurs et gardes-fortifications et si ces personnes étaient porteuses d'un appareil auditif (en tant qu'indicateur d'une acuité auditive réduite) plus fréquemment que la moyenne de la population suisse du sexe masculin. D'abord, on a analysé la série historique de la densité d'incidence des traumatismes acoustiques chez les militaires ainsi que chez les instructeurs de l'armée et gardes-fortifications. Ensuite on a comparé le taux de prévalence pour porteurs d'un appareil auditif chez le instructeurs de l'armée et gardes-fortifications avec celui chez la population suisse du sexe masculin. La densité d'incidence pour traumatismes acoustiques concernant les instructeurs de l'armée et gardes-fortifications observée pendant dix ans ne montrait pas de changement significantif; au contraire, les militaires montraient une forte diminution (−12% par an). Le taux de prévalence pour porteurs d'un appareil auditif était significativement élevé (RR 3.91 [95% Cl 3.09–4.96]) chez les instructeurs de l'armée et gardes-fortifications comparé à la population suisse du sexe masculin. Les résultats parlent en faveur d'un risque élevé pour des lésions de l'ouie chez les instructeurs de l'armée et gardes-fortifications à cause des bruits de tir (maladie professionnelle). Il est recommandé de sousmettre ces personnes à un programme préventif obligatoire.
