126. Erfahrungen mit der Anlage einer kontinenten Gastrostomie

Summary

Using the GIA auto suture instrument, a “continuous gastromy” was performed on 12 patients with nonresectable carcinomas of esophagus or pharynx and with stenosis or fistula of esophagus after cobalt radiation. The advantages of this gastrostomy, performed as a standard procedure based on the idea of Depage, Janeway, and Spivak, over the usual gastrostomy of Witzel or Kader, are: 1. It is unneceBary to open the stomach during the abdominal operation; 2. there is no leakage of gastric juice on the skin surface and no danger of rapid obliteration by adhesion of the serosal surface. The tube need only be inserted for feedings.

ZusammenfaBung

Bei 12 Patienten mit inoperablem Oesophagus-Ca., Pharynx-Ca. sowie OesophaguBtenose oder Fisteln nach Kobaltbestrahlung wurde mit dem GIA-Nähapparat eine sog. kontinente, permanente Gastrostomie angelegt. Die Vorteile dieser Gastrostomie, die in einer standardisierten Technik nach den Vorstellungen von Depage, Janeway u. Spivak entwickelt wurde, sehen wir gegenüber der herkömmlichen Gastrostomie nach Witzel oder Kader in der kurzen Operationsdauer und der geringen Operationsbelastung sowie der Kontinenz der Gastrostomie. Da der Fistelkanal nicht obliteriert, braucht ein Schlauch nur noch zur Einfüllung der Nahrung eingeführt zu werden.
