Current knowledge in cleft lip and palate treatment from an orthodontist's point of view

Aktuelle Behandlungsprinzipien bei Lippen-, Kiefer-und Gaumenspalten aus Sicht des Kieferorthopäden

Abstract

The aim of this review was to put new clinical research findings into proper perspectives relative to previosly accepted knowledge on treatment of patients with cleft lip and palate. The first part of the pape deals with varous aspects of infant orthopedic treatment, such as its influence on primary surgery, maxillary arch form and dimensions, feeding, psychological situation of the parents and speech development. Following parts analyze general maxillofacial growth outcome after surgery and also maxillofacial growth in relation to particular surgical procedures (palatal repair, periosteoplasty/gingivoplasty, bone grafting). The last parts of the review discuss the effects of certain orthodontic/orthopedic treatment approaches as well as the role of dental implants in treatment of cleft lip and palate patients.

Zusammenfassung

Ziel dieser Übersicht ist es, neue Erkenntnisse der klinischen Forschung zu früher akzeptiertem Wissen über die Behandlung von Patienten mit Lippen-Kiefer-Gaumen-Spalten in Beziehung zu setzen. Der erste Teil dieses Beitrags befaßt sich mit den verschiedenen Aspekten der kieferorthopädischen Behandlung im Säuglingsalter, wie zum Beispiel dem Einfluß auf die Primärchirurgie, die Form und Dimension des oberen Zahnbogens, die Nahrungsaufnahme, die psychologische Situation der Eltern und die Sprachentwicklung. Die nachfolgenden Teile analysieren das allgemeine Kiefer-und Gesichtswachstum nach erfolgter Chirurgie sowie das maxillofaziale Wachstum bei speziellen chirurgischen Verfahren (Gaumenverschluß, Periost-und Gingivoplastik, Kieferosteoplastik). Der letzte Teil diskutiert sowohl den Effekt bestimmter orthodontischer bzw. orthopädischer Behandlungskonzepte als auch die Rolle von Zahnimplantaten in der Behandlung von Spaltpatienten.
