Zusammenfassung
In einer Fall-Kontroll-Studie wurden 192 Patienten mit histologisch gesichertem Prostatakarzinom mit einer Kontrollgruppe von 210 Männern verglichen, bei denen das Vorliegen eines Prostatakarzinoms histologisch ausgeschlossen worden war. Fälle und Kontrollpersonen stammen aus zwei urologischen Praxen in Hamburg und Frankfurt/M. Sowie aus der urologischen Poliklinik der Universitätsklinik in Frankfurt/M. Erfragt wurden sämtliche Berufstätigkeiten (von mindestens einem Jahr Dauer), Rauchen und Alkoholkonsum sowie die Exposition mit spezifischen Substanzen. Mittels logistischer Regression wurden für Alter, Erhebungsort und Raucherstatus adjustierte Odds Ratios berechnet. Bei keiner der abgefragten beruflichen Expositionen oder konsumbezogenen Faktoren findet sich ein signifikanter Unterschied zwischen Fall-und Kontrollgruppe. Es ergibt sich ein statistisch signifikanter Zusammenhang zwischen der Betätigung in der Gruppe der Verkehrsberufe und dem Prostatakarzinom (p=0.006). Wird die Dauer der beruflichen Tätigkeit kategorisiert, so ergeben sich im Vergleich mit den niemals in Verkehrsberufen Beschäftigten folgende Odds Ratios (OR): OR=0,6 (95% Cl: 0,3–1,2) für 1–10 Jahre, OR=2,0 (95% Cl: 0,8–4,8) für 11–30 jahre und OR=3,2 (95% Cl: 1,3–7,5) für>30 jahre Ausübung von Berufen dieser Gruppe. Bei den übrigen Berufsgruppen ließen sich keine signifikanten Assoziationen zum Prostatakarzinom finden. Die Ergebnisse liefern weitere Evidenz für einen Zusammenhang zwischen der Tätigkeit in Verkehrsberufen und der Entwicklung eines Prostatakarzinoms.
Summary
In a case-control study 192 patients with histologically confirmed prostate cancer were compared with 210 men in whom carcinoma of the prostate had been histologically excluded. Cases and controls were recruited from two urological practices in Hamburg and Frankfurt/M. and from the urologic policlinic at the University of Frankfurt/M. The probands were questioned about smoking habits and alcohol consumption, about all occupational activities of least one years' duration, and about exposure to specific substances. Odds ratios (OR) were calculated using logistic regression analysis, adjusting for age, region and smoking. There were no differences between patients and controls in selfreported occupational exposures or the consumption of alcohol or smoking. There was a positive association between having worked in transportation/communication and having prostate cancer (p for trend=0.006): ORs varied according to the number of years worked as follows: 1–10 years versus never, OR=0.6 (95% Cl: 0.3–1.2), 11–30 years, OR=2.0 (95% Cl: 0.8–4.8), >30 years OR=3.2 (95% Cl: 1.3–7.5). Work in other occupational groups was not associated with an increased risk for prostate cancer. These results suggest that transport work may be associated with the development of prostate cancer.
Résumé
Dans une étude de cas-témoins 192 patients atteints d'un carcinome prostatique vérifié par examen histologique étaient comparés avec 210 hommes dont une diagnose d'un carcinome prostatique eut exclu. Les cas et les témoins ont été recrutés dans deux cabinets de médecins urologues à Hamburg et Frankfurt/Main ainsi que dans la polyclinique urologique du CHU à Frankfurt/Main. Les cas et les témoins ont été questionnés sur toutes leurs activités, leurs habitudes de consommation ainsi que sur leur exposition professionnelle aux substances specifiques. Les rapports des cotes (OR) ajustés pour l'age, la région, et la consommation de tabac étaient déterminés à travers l'analyse de régression logistique. Il n'y avait pas d'association entre les expositions profes sionnelles ou la consommation de tabac ou l'alcool et le carcinome prostatique. Patients avec le cancer étaient enregistrés dans la tranche de professions de transports et communications plus souvent que les témoins (p=0,006). En comparant les personnes travaillant dans le domaine des transports avec celles d'autres domaines professionnelles, les OR ont varié selon les années d'occupation comme suit: OR=0.6 (95% Cl: 0,3–1,2) pour 1 à 10 ans, OR=2,0 (95% Cl: 0,8–4,8) pour 11 à 30 ans, OR=3,2 (95% Cl: 1,3–7,5) pour plus de 30 ans. Chez les autres groupes professionnelles aucune différence pouvait être constatée entre les cas et les témoins. Les résultats de cette étude mettent en évidence qu'il existe un certain risque d'être atteint d'un carcinome prostatique pour des personnes travaillant dans le domaine des transports.
