14. Stapler versus handgenähte Anastomose in der Magen-Darm-Chirurgie

Automatic staplers versus hand-sutured anastomoses in gastrointestinal surgery

Summary

The comparative effectiveness of manual and auto-sutured anastomoses after total gastrectomy (107 cases), low anterior rectal resection (100 cases) and right hemicolectomy (34 cases) is studied. In the stapled series the incidence of anastomotic leakage was somewhat less, although benign stenosis occurred more frequently than with hand-sutured anastomoses. However, this could not be statistically demonstrated. In the authors' view, the main advantages of the use of staplers are the shortening of operating time and the lessening of septic steps.

Zusammenfassung

Nach totaler Gastrektomie (107 Fälle), tiefen vorderen Rectumresektionen (100 Fälle) und Rechtshemicolektomie (34 Fälle) wurde die Effektivität der manuellen und maschinellen Anastomosen verglichen. In der Staplerserie ist die Anastomoseninsuffizienzhäufigkeit etwas niedriger, obwohl die benignen Stenosen häufiger gegenüber den handgenähten Anastomosen auftreten. Statistisch aber ist dies nicht beweisbar. Unserer Meinung nach sind die Vorteile der Anwendung der zirkulären Nähapparate: Verkürzung der Operationszeit und Reduktion der septischen Operationsphasen.
