Summary
The different causes of cardiogenic shock and circulatory failure have to be treated according to their aetiology and the state of the patient. Recent experience with percutaneous transfemoral valvuloplasty in 33 patients with aortic stenosis and angioplasty of occluded coronary arteries in acute postoperative myocardial infarction in 13 patients is in favour of active treatment of patients before and even after circulatory failure has occurred. Aortic valvuloplasty was performed successfully and with minor complications in 5 patients in cardiogenic shock.
Zusammenfassung
Die Ursachen und der Zustand des Patienten mit kardiozirkulatorischem Versagen bestimmen die therapeutischen Möglichkeiten. Unsere aktuellen günstigen Erfahrungen mit der percutanen transfemoralen Valvuloplastie der stenosierten Aortenklappe bei 33 Kranken prä- und postoperativ, mit und ohne bereits eingetretener Dekompensation lassen diesen Eingriff als Therapie der Wahl erscheinen. Gleichfalls sollte beim akuten postoperativen Herzinfarkt der sofortige Versuch der mechanischen Reperfusion des verschlossenen Coronargefässes gemacht werden. Bei 8 von 13 Patienten liess sich das Gefäss wieder eröffnen.
