Summary

Between 1964 and 1979 a total of 1206 reconstructive operations for aortoiliac obstructions were performed: the retroperitoneal approach (R.P.A.) in 561 patients (47%), the intraperitoneal approach in 428 cases (35%), and extra-anatomical techniques in 217 cases (18%). During the last fifteen years the percentage of R.P.A. has remained approximately the same (range of 50%). In our practice the R.P.A. is the procedure of choice to carry out safely endarterectomy of the aortoiliac toboggan. The review of our first 100 cases operated on before 1970 over 10 years follow-up demonstrated good results: the permeability rate over 10 years is 90% for stage 11, 70% for stage 111, 50% for stage IV.
