Summary
The following methods to prevent rejection crises during the first year after transplantation are discussed and tested: 1. Matching in HLA-D and DR antigens seems to be more important than in HLA-A, B, or C antigens. 2. One or several blood transfusions prior to transplantation improve transplant acceptance, although the mechanism of this phenomenon is still unknown. 3. Cyclosporin A, a fungus-metabolite, seems to be a strong immunosuppressive drug with special effects on cellular immune reactions. 4. Total lymphoid irradiation, leaving other tissues blank, is able to suppress rejection crises without additional immunosuppression.
Zusammenfassung
Zur Überwindung von Abstoßungskrisen innerhalb des ersten Jahres nach Transplantation werden folgende Methoden diskutiert und erprobt: 1. Übereinstimmung in den HLA-D und -DR-Antigenen scheint wichtiger zu sein als in den HLA-A-, -B- und -C-Antigenen. 2. Eine oder mehrere Bluttransfusionen vor Transplantation begünstigen die Transplantatakzeptanz; Mechanismus noch nicht geklärt. 3. Cyclosporin A, ein Metabolit von Pilzen, scheint ein neues und starkes, die celluläre Immunreaktion unterdrückendes Medikament zu sein. 4. Durch totale Lymphgewebebestrahlung, unter Aussparung anderer Gewebe, konnte in Experiment und Klinik die Abstoßung ohne weitere zusätzliche immunsuppressive Maßnahmen unterdrückt werden.
