Summary
Our analysis was based on 172 cases of brain death and on the international literature. The prerequisites are: clear diagnosis, in particular of direct brain damage with acute intracranial hypertension but no intoxication; hypothermia; or shock. The clinical symptoms are: unresponsive coma, apnoe in spite of intubation, bilateral fixed mydriasis, and absence of other brainstem reflexes. The time necessary for diagnosis varies from 1–24 h, usually 6–12. Early diagnosis is possible with 30 min isoelectric EEG and/or angiographic evidence of arrest of cerebral circulation.

Eigene Beobachtungen bei 172 Fällen und Analyse der Literatur ergeben: UnverzichtbareVoraussetzungen: eindeutige Diagnose, vor allem direkte Hirnschädigung mit akuter intrakranieller Drucksteigerung; Ausschluß von Intoxikation, Hyperthermie und Schock. KlinischeSymptome: Koma, Apnoe trotz Intubation, beiderseits lichtstarre Mydriasis und Ausfall anderer Hirnstamm-Reflexe.Zeit-Dauer bis Diagnose-Stellung: unterschiedlich 1–24, meistens 6–12h. Frühe Diagnose durch Null-Linien-EEG über 30 min und/oder cerebraler Zirkulationsstillstand bei der Angiographie.
