Summary
The first successful keratoplasty was performed at the beginning of this century. Antibiotics, immunosuppressives, and microsurgery led to a remarkable improvement of the success rate following corneal grafting. About 1200 keratoplasties are performed every year in the FRG. Of the grafts, 85%–88% stay optically clear. The rate of complications is low (1–2 per 1000 grafts). An immunological reaction occurs in about 5-10 %. The indications for corneal grafting are: corneal dystrophies, corneal burns, and optical corrections. Future trends are: improved donor selection (corneal microscopy, tissue antigen) and keratoprosthesis.
Zusammenfassung
Die erste erfolgreiche Keratoplastik erfolgte zu Beginn des 20. Jahrhunderts. Antibiotica, Mikrochirurgie, Immunsuppressiva ermoglichten in den vergangenen 20 Jahren eine ungewohnliche Verbesserung der Transplantations-Resultate. Derzeit werden anndhernd 1200 Keratoplastiken pro Jahr in der Bundesrepublik Deutschland durchgeführt. Klares Einheilen in 85–88%. Die Komplikationsrate ist klein (ca. 1–2 auf 1000). Transplantat-Reaktionen (maladie du greffon) treten in ca. 5–10% auf. Indikationen zur Keratoplastik: Hornhautdystrophie, Hornhautveratzung, optische Korrekturen. Tendenzen: Verbesserte Auswahl des Spendermaterials (Gewebekompatibilitat, Hornhautendothel-mikroskopie. Kunststoff-Keratoprothese.
