Summary

Pancreatic transplantation is indicated only in juvenile diabetes with terminal renal failure needing renal grafting. Two procedures are evaluated, organ transplantation and islet transplantation. In organ transplantation, complications caused by the exocrine function are avoided by intraductal injection of a solidifying substance or by intraperitoneal drainage of the juice. Islet transplantation is technically standardized and free of complications, but mostly it is unsuccessful because of early, up-to-now poorly understood rejection. In only one case has long-term function of human pancreatic islets been achieved.

Zusammenfassung

Die Pankreastransplantation ist nur beim juvenilen Diabetes mit Niereninsuffizienz indiziert, der auch der Nierentransplantation bedarf. Zwei grundverschiedene Verfahren stehen in Entwicklung. Bei der Transplantation eines intakten Organsegments wird zur Vermeidung von Komplikationen seitens der exokrinen Funktion das Gangsystem mit einer schnellhärtenden Substanz ausgegossen oder ein freier intraperitonealer Sekretabfluß geschaffen. Die Inseltransplantation andererseits ist technisch standardisiert and komplikationsfrei, scheiterte bisher aber fast immer an einer frühen, in ihrem Wesen noch nicht geklärten Abstoßung. Bisher konnte erst einmal eine Langzeitfunktion erzielt werden.
