26. Die plastisch-chirurgische Deckung von Strahlenulcera im Thorax- und Beckenbereich

Summary

Ulcerations after postoperative irradiation of the chest wall and the pelvic area are very painful and immobilizing; they also have a very unpleasant odor. In a total of 163 patients with ulcers in the supra-clavicular groove, the axilla, chest wall, groin, sacrum, and genital area, the ulcers were treated by wide and deep excision and subsequent closure of the defect with local skin flaps. For deeper defects of the thoracic wall and the lower extremities, myocutaneous flaps (latissimus dorsi, gracilis) are the method of choice.

Zusammenfassung

Strahlenulcera im Thorax- und Beckenbereich sind meistens sehr schmerzhaft, bewegungseinschränkend und stinkend. 163 Patienten mit Ulcerationen am Hals, in der Axilla, an der Thoraxwand, Leiste, über dem Sacrum und im Genitalbereich wurden nach weiter und tiefer Excision mit lokalen Schwenklappen versorgt. Bei tiefen Defekten der Thoraxwand und der unteren Extremität sind myocutane Lappen (Latissimus dorsi, gracilis) die Methode der Wahl.
