16. Chemotherapie und palliative Therapie des Bronchialcarcinoms

Summary

This report summarizes our experience with protocols ,ACO I" and ,ACO II" and with alternative combinations such as ifosfamide plus etoposide in the chemotherapy of inoperable bronchogenic carcinoma. Using the „ACO”-protocols and consolidating irradiation, 77% complete remissions were achieved in patients with limited disease (median survival of 21 months). Patients with extensive disease (50 % complete remissions) had a median survival of 10 months. The survival data point to a plateau after 30 months, suggesting that approximately 20 % of limited disease patients may achieve long-term survival. Relapse patterns are analyzed.

Zusammenfassung

Es wird über die Chemotherapie bei inoperablen kleinzelligen Bronchial carcinomen unter Verwendung des ACO-Protokolls (Adriamycin, Cyclophosphamid und Vincristin) sowie der Kombination Ifosfamid und VP 16–213 sowie weiterer Chemotherapie Verfahren berichtet. Mit dem ACO-I und ACO-II-Protokollen ließen sich bei Patienten mit limitierter inoperabler Erkrankung 77 % Vollremissionen, bei extendierter Erkrankung 50 Vollremissionen erzielen. Bei limitierter Erkrankung betrug die mittlere Überlebensdauer 21 Monate. Nach 30 Monaten prägt sich ein Plateau auf, so daß etwa mit 20 % Langzeitüberlebenden zu rechnen ist. Das Rezidivverhalten beider Gruppen wird dargestellt.
