27. Zweitoperationen an den Gallenwegen —Indikationen und Techniken

Summary

Recurrent choledocholithiasis, stenosis of the sphincter of Oddi, contraindication to choledochoduodenostomy, and stricture of the common duct are the main indications for reoperations. Probing of the papilla of Vater via duodenoscopy and retrograde cholangiography is a helpful diagnostic procedure. Technically there are two different situations: a) cases in which the normal biliary tract can be preserved or reconstructed, which is always preferable if technically possible; b) end-to-end reconstruction, which is possible practically only after fresh injuries. Decompressive fistulas should not be constructed from the duodenum but from a Roux-Y loop of the jejunum.

Zusammenfassung

Rezidivsteine, Papillenstenose, fälschlich angelegte Choledochoduodenostomie und Strikturen sind für Nachoperationen die häufigsten Indikationen, fdr die heute die duodenoskopische Papillensondierung mit retrograder Gallengangsdarstellung besonders wertvolle Hinweise gibt. Operationstechnisch sind zu unterscheiden a) die Fälle, bei denen die normalen anatomischen und Abflußverhiiltnisse erhalten bzw. wiederhergestellt werden können; dies ist, wenn technisch möglich, immer vorzuziehen. b) Rekonstruktionen sind End-zuEnd fast nur bei frischen Verletzungen möglich. Biliodigestive Anastomosen sollten nicht mit dem Duodenum, sondern nur mit einer Y-förmig ausgeschalteten hohen Jejunumschlinge hergestellt werden.
