10. Operative Therapie retroperitonealer Tumoren

Summary

The diagnosis of retroperitoneal tumors is usually made too late because of the lack of early symptoms. They are recognized after compression of the bowel, the kidneys and the ureter or the great vessels. In only 20–25% is radical surgery possible. A recurrence rate of 25–50% is reported. We prefer the transperitoneal approach, so that the operation can be extended into the thorax when necessary. A biospy must be taken if the tumor cannot be removed. At the Chirurgische Universitätsklinik Mainz, 62 patients with retroperitoneal tumors were treated from 1964–1972. Of these patients, 56 had malignant and 6 benign tumors. Radical surgery was possible in 11 cases of malignancy. Five of the patients have so far survived for 5 years after surgery.

Zusammenfassung

Retroperitoneale Tumoren gelangen aufgrund ihrer Symptomlosigkeit zu spät in operative Behandlung. Sie werden erst durch Kompressionserscheinungen auf Darm, Niere, Ureter und große Gefäße erkannt. Trotz Fortschritten in der Diagnostik sind nur 20–25% operabel. Rezidivquote: 25–50%. Dennoch stellt die Operation mit dem Ziel den Tumor radikal zu entfernen das Verfahren der Wahl dar. Der ventrale transperitoneale Zugang, fallweise mit abdomino-thorakaler Erweiterung, schafft die Voraussetzung zu großen Radikaloperationen. Auch bei Inoperabilität muß durch Probeexcision die Artdiagnose gesichert werden.

An der Chir. Univ.-Klinik Mainz wurden von 1964–1972 insgesamt 62 retroperitoneale Tumoren (56 bösartig, 6 gutartig) operiert, 11 Malignome waren operabel. 5 Patienten leben länger als 5 Jahre.
