Summary
The introduction of the operative microscope in the surgical treatment of peripheral nerve injuries can be considered as a real progress. The operative microscope permits an accurate estimate of the nature and the extend of a peripheral nerve lesion. Using this instrument it was succeeded to dissect the peripheral nerve in more fine anatomic units. Following cleavage and removal of the epineurium it is possible to expose the individual nerve fascicles with their ven perineurium. In order to recognize a nerve lesion precisely involving a fibrosis of the epineurium as well as a strangulation of the individual fascicles a higher optic magnification is needed. If individual fascicles have to be liberated from the scar tissue we have to count with a decompressive neurolysis. This technique which is designated as “fascicular perineural neurolysis” is possible to be performed only under the operative microscope. Moreover through the microtechnique the nerve suture has become finer. Following preparation of the nerve stump in smaller fascicles respectively groups of fascicles, a more accurate addaptation of the nerve truncs through perineural sutures will essentially be performed.
Also the technical problem of the autologous nerve grafting has been further resolved following the application of the operative microscope. The difference in diameter between the nerve trunc which has to be repaired and the thin nerve of the skin do not play an important role any more. The individual fascicles respectively the groups of fascicles of the nerve stump can microsurgically be prepared in such a way as to correspond with the diameter of the excided autograft.
Zusammenfassung
Als ein echter Fortschritt kann die Einführung des Operationsmikroskops in die chirurgische Versorgung peripherer Nervenverletzungen angesehen werden. Das Operationsmikroskop erlaubt eine sichere Beurteilung der Art und des Ausmaßes einer Läsion peripherer Nerven. Durch das Operationsmikroskop ist es gelungen, die peripheren Nerven in feinere anatomische Einheiten zu präparieren. Nach Spaltung und Entfernung des Epineuriums können die einzelnen Nervenfaszikel mit ihrem glatten Perineurium dargestellt werden. Eine Nervenschädigung mit lediglich einer Fibrose des Epineuriums und Strangulation der einzelnen Faszikel kann nur unter starker Vergrößerung erkannt werden. Mit einer Dekompression durch eine Neurolyse ist dann zu rechnen, wenn die einzelnen Faszikel von dem Narbengewebe befreit werden. Diese Technik, die wir als faszikuläre perineurale Neurolyse bezeichnen, kann nur unter dem Operationsmikroskop durchgeführt werden. Auch die Nervennaht hat sich durch die Mikrotechnik weitgehend verfeinert. Nach Präparation der Nervenstümpfe in kleine Faszikel bzw. Faszikelgruppen wird eine wesentlich exaktere Adaptation der Nervenstümpfe durch perineurale Nähte erreicht. Auch das technische Problem der autologen Nerventransplantation wurde durch die Einführung des Operationsmikroskopes weitgehend gelöst. Die Kaliberunterschiede zwischen wiederherzustellendem Nervenstamm und dünnen Hautnerven spielen heute keine Rolle mehr. Die einzelnen Faszikel bzw. Faszikelgruppen der Nervenstümpfe können mikrochirurgisch so hergestellt werden, daß sie dem Durchmesser des entnommenen Transplantates entsprechen.
