Summary
The requirement of calories has to be fulfilled primarily to achieve nitrogen balances ranging from equalized to positive. Basic requirement per kg bodyweight and per day 1 g of amino acids, 2 g of fat, 2 g of carbohydrates. Additional requirements resulting from operation, fever, etc. have to be taken into consideration. 35–60 Kcal per kg bodyweight and day may be necessary.
Supply of Nitrogen. Supply of amino acids does not represent any problems. Requirement amounts to 240–480 mg N/kg bodyweight/day corresponding to 1,5–3,0 proteins per kg bodyweight/day. Amino acids should be infused simultaneously with carbohydrates or fat.
Fat. Fat is indispensable for the purpose of fulfilling the calorical requirement within a certain volume. Products of Sojabeanoil are tolerated well, disagreements seldom occur. Fat should be infused slowly, depending on concentration 500 ml within 3–6 h. In case of continuous parenteral nutrition infusion has to be made by Cava-Catheter. The kind of pigmentation of the liver ist unknown yet.
Carbohydrates. Glucose, fructose, sorbit or xylit can be infused easily, all carbohydrates used in large amounts may represent problems. Mixtures e.g. glucose and fructose should therefore be preferred. During stress metabolized carbohydrates not depending on insuline are preferred. 100 g Kh = 400 Kcal suffice to prevent an acidose.
In case of continuous complete parenteral nutrition supply of electrolytes, trace elements and vitamines has to be guaranteed.
Zusammenfassung
Die Deckung des calorischen Bedarfes ist vordringlich, um eine ausgeglichene bis positive Stickstoffbilanz zu erreichen. Basalbedarf pro kg Körpergewicht und Tag: 1 g Aminosäuren, 2 g Fett, 2 g Kohlenhydrate. Mehrbedarf durch Operation, Fieber usw. ist zu berücksichtigen. Notwendig können 35–60 Kcal pro kg Körpergewicht und Tag sein.
Stickstoffzufuhr. Die Zufuhr an Aminosären ist unproblematisch. Der Bedarf beträgt 240–480 mg N/kg Körpergewicht/Tag, entsprechend 1,5–3,0 g Eiweiß pro kg Körpergewicht/Tag. Infusion der Aminosfäruren simultan mit Kohlenhydraten oder Fett.
Fett. Das Fett ist unentbehrlich, um den calorischen Bedarf in einem begrenzten Volumen zu befriedigen. Die Sojaölzubereitungen werden gut toleriert, Unverträglichkeitserscheinungen treten kaum noch auf. Das Fett muß langsam infundiert werden, je nach Konzentration 500 ml in 3–6 Std. Bei länger dauernder parenteraler Ernährung Zufuhr durch den Cava-Katheter. Bedeutung der Pigmentierung der Leber noch ungeklärt.
Kohlenhydrate. Glucose, Fructose, Sorbit oder Xylit lassen sich mühelos infundieren, alle Kohlenhydrate sind in größeren Mengen nicht unproblematisch. Mischungen, z. B. Glucose und Fructose, sind daher vorzuziehen. Insulinunabhängig abgebaute Kohlenhydrate sind im Stress bevorzugt, 100 g Kh = 400 Kcal reichen aus, um als Minimalcalorienzufuhr eine Acidose zu verhindern. Bei länger dauernder, vollständiger parenteraler Ernährung müssen Elektrolyt-, Spurenelement- und Vitaminzufuhr sichergestellt werden.
