Summary
The sociological importance of ulcers has often been stressed. After exhausting the possibilities of conservative treatment, the operative therapy becomes necessary. Here, the main goals are low mortality and relapse risk, and maintenance of stomach function.
These criteria and the increasing knowledge of ulcerogenic factors have tended to displace the 2/3- or 3/4-resection with Billroth's Second Method, from a position of importance as far as optimal treatment is concerned. On the other hand, completely relinquishing the resection method requires a subtle secretion analysis, as well as knowledge of the prospective index of reduction. The average values contain a degree of uncertainty. Furthermore, vagotomia alone will not cancel out the extra vagal mechanisms of stimulation, of course. Therefore, we prefer the Combined Operation (Harkins and Nyhus). The mortality rate lies in the range of 0.7, the relapse quota 1.2 and the occupational fitness (occupation previous to onset of disease) 92%.
Zusammenfassung
Das Ulcus ist ein soziologisch bedeutsames Leiden. Nach Erschöpfung konservativer Behandlungsversuche erfordert es die operative Therapie. Dabei sind geringe Letalität, Rezidivfreiheit und Erhaltung der Magenfunktion anzustreben. Diese Postulate und die zunehmende Kenntnis der ulcusgenetischen Faktoren haben inzwischen die 2/3- oder 3/4-Resektion von B II von ihrer Position als Optimalverfahren verdrängt. Andererseits hat ein völliger Verzicht auf die Resektion eine subtile Sekretanalyse mit Kenntnis des prospektiven Reduktionsindex zur Voraussetzung. In den Mittelwerten bleiben hier jedoch Unsicherheiten, ebenso wie extravagale Stimulationsmechanismen mit der Vagotomie allein nicht erfaßbar sind. Deshalb geben wir der kombinierten Operation (Harkins u. Nyhus) den Vorzug. Die Letalität liegt bei 0,7%, die Rezidivquote bei 1,2%, die Berufsfähigkeit (vor Beginn des Leidens ausgeübter Beruf) bei 92%.
