11. Herzchirurgie

Summary

Based on a review of the last ten years only a small selection is described. The developments in the field of extracorporal and assisted circulation are touched, as well as those in the surgery of the most important complex cardiovascular deformities and questions of heart-valve replacements and coronary surgery. The theme “rtificial heart” is left out, because its clinical relevance is not given for the present. A position towards heart transplant is not taken, because a particular report of the surgery of transplant is assigned. In the scope of extracorporeal circulation for routine application in numeros cases the method of hemodilution in connection with disposable-foam-oxygenators has brought a real breakthrough.

For prolonged perfusion, membrane-oxygenators promise great advantages, which now come hesitatingly to their clinical use. The application of real pulsatile blood pumps is still problematic. In the field of assisted circulation the counterpulsation with the intra-aortic balloon-technique plays the most important part as well as the partial by-pass with membrane-oxygenators and blood pumps. With the latter technique even in prolonged perfusions of several days first successes were achieved. In the scope of surgical attendance of cyanotic deformities successes are to register by the better coordination of more effective palliative operations in the sense of operations at two different timings. Coronary surgery has after the refinement of coronary arteriographic techniques now reached the stage of the clinical test of direct methods for revascularisation.[/p]

Concluding it is pointed out that structural problems are responsible for the difficulties concerning the quantitative provision of the population of the Federal Republic with operations in cardiac anomalies and acquired cardiac lesions. The attention is called to a way for the solution of this problem.

Zusammenfassung

Unter Zugrundelegung eines Berichtszeitraumes der letztvergangenen 10 Jahre wird eine sehr enge Auswahl getroffen. Es werden die Entwicklungen auf dem Gebiet der extrakorporalen und assistierten Zirkulation, der Chirurgie der wichtigsten komplexen kardiovaskulären Mißbildungen, Fragen des Herzklappenersatzes und der Coronarchirurgie gestreift. Das Thema „Künstliches Herz” wird ausgeklammert wegen seiner noch nicht gegebenen klinischen Relevanz. Zu Fragen der Herztransplantation wird nicht Stellung genommen, da für die Transplantationschirurgie ein eigenes Referat vorgesehen ist.[/p]

Im Rahmen der extrakorporalen Zirkulation hat für die Routineanwendung in sehr zahlreichen klinischen Fällen die Blutverdünnungsmethode bei gleichzeitiger Anwendung von Einmal-Schaum-Oxygenatoren zu einem echten Durchbruch geführt. Für Langzeitperfusionen versprechen die erst zögernd zu klinischem Einsatz kommenden Membran-Oxygenatoren große Vorteile. Die Anwendung echt pulsatiler Blutpumpen bleibt zunächst noch problematisch.[/p]

Bei der assistierten Zirkulation spielen klinisch die Gegenpulsation mit der intra-aortalen Ballontechnik und der partielle By-pass mit Hilfe von Membranoxygenatoren und Blutpumpen zur Zeit die Hauptrolle. Mit der letztgenannten Technik sind auch bei mehrtägiger Perfusion erste Erfolge erzielt worden.[/p]

Im Rahmen der chirurgischen Behandlung cyanotischer Mißbildungen sind Erfolge zu verzeichnen durch die bessere Zuordnung effektiverer Palliativ-Operationen im Sinne einer zweizeitigen operativen Behandlung. - Die Coronarchirurgie ist nach Verfeinerung röntgen-diagnostischer Techniken in das Stadium der klinischen Erprobung direkter Revaskularisationsmaßnahmen getreten.[/p]

Abschließend wird auf die Ursachen der Schwierigkeit der quantitativen herzchirurgischen Versorgung der Bevölkerung der Bundesrepublik hingewiesen und auf einen Weg zur Lösung des Problems aufmerksam gemacht.
