15. Fisteln im Bereich des Dünndarms, des Dickdarms und des Anorectums

Summary

102 cases are discussed. At diverticulitis of the colon is the most frequent type of fistula, then colitis, fistulas following operations, fistulas forming due to tumor breaking in, and finally fistulas following X-ray irradiation. If acute shock develops in the patient with fistula, immediate relaparotomy is indicated. Location of the fistula decides whether it is to be by-passed or eliminated or whether for instance, a reresection of the small intestine should be carried out. Initially a chronic fistula should be treated conservatively, and surgery should subsequently be applied as in an emergency case.

Zusammenfassung

102 Fälle werden besprochen. An der Spitze steht die Diverticulitis coli, dann kommt die Colitis, die postoperative Fistel, fistelnde Tumoreinbrüche und am Schluß die Fisteln nach Röntgenbestrahlungen. Kranke, deren Fistel mit einem akuten Schock auftritt, sind sofort zu relaparotomieren. Das Fistelgebiet ist je nach Lage, zu umgehen, auszuschalten oder es muß z.B. am Dünndarm eine Nachresektion erfolgen. Chronische Fisteln sind zuerst konservativ und dann individualisierend wie akute Fälle zu operieren.
