Terminolaterale Gastroduodenostomie

Summary

The experiences with 119 cases of end-to-side gastroduodenal anastomosis are reported. The standardized procedure consisted of a Shoemaker type gastric resection with formation of a greater curvature tube and an anastomosis with the duodenum in an oblique position. Details of the operative technique are presented. The procedure as pointed out permits a broader application of the Billroth I type gastric resection. The overall postoperative mortality was 3.4 %, in elective operations 1.7 %. Complications occurred in 11.6 % of the patients. End-to-side gastroduodenal anastomosis above, at the level, or below the duodenal papilla has assumed with us a definite place in gastric resections.

Zusammenfassung

Es wird über 119 Patienten berichtet, bei denen eine terminolaterale Gastroduodenostomie durchgeführt wurde. Einheitlich erfolgte jeweils eine stufenförmige Resektion des Magens, eine Schlauchbildung aus der Groß-kurvatur und eine schräge Anastomose mit dem Duodenalrohr. Das technische Vorgehen wird detailliert geschildert. Die dargestellte Methode erlaubt eine breitere Skala der Indikationsstellung zur Durchführung eines B-I. Die postoperative Letalität beträgt für alle Eingriffe 3,4 %, für die planmäßigen Operationen 1,7 %. Postoperative Störungen wurden bei 11,6 % der Operierten registriert. Die Gastroduodenostomia oralis partialis inferior terminolateralis supra-, contra-, sive infrapapillaris nimmt unter den Möglichkeiten zur Methodenwahl der Magenresektion bei uns einen definierten Platz ein.
