39. Zur Problematik des intraoperativen Schnellschnitts bei Neoplasien der Schilddrüse und des chirurgischen Vorgehens beim atypischen Adenom

Summary

For a differential diagnosis of atypical adenoma and highly differentiated follicular carcinoma cryosections are of value only if findings are positive. Between 1977 and 1982 we observed 85 thyroid carcinomas and 47 atypical adenomas. In 5 cases the diagnosis on the basis of a cryosection had failed to detect a carcinoma. In patients with atypical adenoma we performed a subtotal resection; a primary lobectomy was not carried out as it would have meant exposing an unnecessarily high percentage of patients to an increased risk of complications. 3-8 years postoperatively the patients show no pathological findings.

Zusammenfassung

Für die Differentialdiagnose atypisches Adenom und hochdifferenziertes folliculäres Carcinom steht dem Schnellschnitt nur im positiven Fall Aussagekraft zu. 1977–1982 beobachteten wir 85 Schilddrüsencarcinome und 47 atypische Adenome, 5mal mußte die Schnellschnittdiagnose nachträglich in Richtung Carcinom revidiert werden. Beim atypischen Adenom führten wir eine subtotale Resektion aus und verzichteten auf die primäre Lobektomie, weil ein unverhältnismäßig hoher Prozentsatz von Patienten unnotwendigerweise ihrem erhöhten Komplikationsrisiko ausgesetzt worden wäre. Die 47 Patienten sind 3-8 Jahre postoperativ o.B.
