29. Wann ist eine simultane Operation des Meckel'schen Divertikels indiziert: (Immer - niemals - oder nur unter besonderen Voraussetzungen?)

Meckel's diverticulum causes in complications 4–16%. These appear, however, only in adolescents. Thus the simultaneous removal of a diverticulum without symptoms is only justified in adolescents. One should renounce to a search and removal in adults. In case of appendectomy indicated by suspected appendicitis with the appendix found to be normal and absent exsudation in the abdomen, the search for a Meckel's diverticulum is superfluous. One should also renounce to a search and removal if there is a septic abdominal disease, because a removal in peritonitis is more complicated than the risk of complications a not-removed diverticulum will cause.

Zusammenfassung

Meckel'sche Divertikel machen in 4–16% Komplikationen. Diese kommen aber ausschließlich bei Jugendlichen vor. Eine Berechtigung zur simultanen Abtragung symptomfreier Divertikel liegt darum nur bei Jugendlichen vor. Auf eine Fahndung und Abtragung bei Alten ist zu verzichten. Bei einer Appendektomie wegen Appendicitis-Verdacht mit negativem Befund an der Appendix und fehlendem Exsudat im Abdomen, ist die Suche nach einem Meckel'schen Divertikel überflüssig. Auf eine Fahndung und Abtragung sollte auch verzichtet werden, wenn eine septische Baucherkrankung vorliegt, weil eine Abtragung in einer Peritonitis komplikationsreicher ist, als das Komplikationsrisiko eines belassenen Divertikels.
