28. Simultaneingriffe im Bereich der Gallenwege und eines intraabdominalen Organs: Was ist wann indiziert?

Summary

405 surgical interventions had been carried out simultaneously on the biliary tract and other intraperitoneal organs from 1960 to 1984. The patients were divided into three groups. Concerning group A and B (58 patients) the indication for that doubleoperation was absolute. The range of postoperative wound infections was 10.3%, complications were noted in 24.1%. Lethality rate was 6.8%, hospitalisation took place for 17.7 days. Concerning group C (347 patients) without absolute indication for surgical treatment on the biliary tract and another intraperitoneal organ wound infections subsequent to operating occurred in 5.7%, lethality rate was 2.3%. Complications could be seen in 12.1% and hospital treatment continued for 15.5 days. Principally occasional operations and surgical treatment carried out simultaneously should be avoided.

Zusammenfassung

Von 1960–84 wurden bei uns 405 Simultaneingriffe im Bereich der Gallenwege und eines anderen intraperitonealen Organs durchgeführt. Die Patienten wurden in drei Gruppen eingeteilt. In den Gruppen A und B (58 Patienten) bestand eine absolute Indikation zum Doppeleingriff. Die Rate der postop. Wundheilungsstörung lag bei 10,3%, die Komplikationsrate bei 24,1%. Die Klinikletalität betrug 6,8%, die Verweildauer 17,7 Tage. In der Gruppe der nicht absolut indizierten Doppeleingriffe (Gruppe C = 347 Patienten) betrug die Rate der Wundheilungsstörung 5,7%, die Letalität lag bei 2,3%. Die Komplikationsrate betrug 12,1%, die Verweildauer 15,5 Tage. Grundsätzlich sollten Gelegenheitsoperationen und nicht absolut indizierte Simultaneingriffe unterbleiben.
