8. Indikation zur prophylaktischen Sklerosierung von Oesophagusvaricen

Summary

In 72 patients with oesophageal varices the intravascular oesophageal variceal pressure was measured, in 31 of them the portal pressure as well. The portal pressure was identical with the variceal pressure only at the cardia. There was a positive relationship between the size of varices and the variceal pressure and between the size of varices and the occurrence of bleeding from varices. Prophylactic endoscopic sclerotherapy is indicated in grade II and III varices.

Zusammenfassung

Bei 72 Patienten mit ÖV wurde endoskopisch der intravasale ÖVDruck bestimmt, bei 31 von ihnen zusätzlich auch der Pfortaderdruck. Nur im Bereich des oesophagokardialen Übergangs stimmen Pfortaderdruck und ÖV-Druck überein. Je größer die ÖV sind, desto höher ist der intravasale ÖV-Druck. Ebenfalls steigt die Blutungsgefahr mit der Größe der ÖV. Eine prophylaktische Sklerosierung sollte bei zweit- und drittgradigen ÖV erfolgen.
