Über die ursache der appendicitis

Summary

Destructive inflammation is caused by poor blood supply of the involved region. The gastrointestinal tract heavily laden with bacteria is normally protected against this, as it is excellently blood-supplied and even remains so, despite its naturally considerably change in volume. In contrast to this, the appendix is filled with lymphatic tissue tending to inflammation; however, it cannot change in volume and blood is only supplied by a single terminal artery without arcads. Investigations into the structure of the muscle and connective tissue show that there is the reason for its constance in shape. The appendix is inextensible. An inflammation arising in the appendix therefore suppresses itself, thus causing the entire organ to perish. This is destructive appendicitis.

Zusammenfassung

Anatomische Untersuchungsergebnisse beantworten die Frage, warum in dem langen, keimbeladenen Magen-Darmkanal gerade die Appendix zu einer gangräneszierenden Entzündung neigt. Die Appendix vermiformis hat eine vom Caecum unterschiedene Muskel- und Bindegewebsfaserarchitektur. Das Caecum kann sich durch Umstellung seiner scherengitterartig gelagerten Fasern im Bereich der Muscularis mucosae und des die dehnbaren Muskelfasern einfassenden Bindegewebes erweitern. Die Appendix dagegen ist erweiterungsunfähig. Ihre Faserarchitektur ist schon beim Kind, aber noch nicht beim Neonatus, so eingerichtet, daß eine Volumenzunahme unmöglich ist. Niemand hat selbst bei einem Ileus eine geblähte Appendix gesehen. Dazu kommt der ausgedehnte Blutgefäßbaum des Darms im Gegensatz zum spärlich mit Blut versorgten Wurmfortsatz, endlich die im Engraum Appendix dicht gelagerten Lymphfollikel im Gegensatz zu den in der faltigen Mucosa liegenden Lympharealen des Caecums. Die Appendix kann sich aus anatomischen Gründen keine starke Entzündung leisten, ohne ihr zu erliegen.
