Summary
Disturbances of liver function following portasystemic anastomoses are mainly due to impaired liver microcirulation and additional deprivation of oxygen. Hemodynamic measurements have failed to determine the degree of circulatory impairment and oxygen deficiency after portacaval shunting. In contrast, application of Platinum multiwire electrodes provides the possibility to determine the degree of circulatory impairment according to the oxygen content of the liver tissue. Male Lewis rats underwent operations of five different types of portasystemic anastomoses and the oxygen content was measured from the liver surface in animals with a normal and a cirrhotic liver. Portacaval end-to-side shunt as well as portacaval transposition resulted in a marked deficiency of oxygen supply only compensated in a normal liver. A side-to-side shunt significantly improved oxygen supply especially in the cirrhotic liver. Splenocaval and mesentericocaval shunting only partially compensated for a fall in oxygen content but prevented a critical O2 deprivation. According to the oxygen supply in the latter shunting procedures it could be demonstrated, that a significant factor for maintaining sufficient oxygen supply to the liver tissue is not the portal blood volume rather than the persistent drainage of pancreatic blood into the liver. A synergistic effect of hepatotrophic factors and vasoactiv substances from a gastrointestinal source should be discussed. The clinical application of microcirculatory diagnostics could gain considerable influence on the choice of the type of the portacaval shunt to be performed in liver cirrhosis associated with portal hypertension.
Zusammenfassung
Störungen der Leberfunktion nach portosystemischen Anastomosen können als Folge erheblich gestörter Mikrozirkulation und zusätzlichem Sauerstoffentzug angesehen werden. Der Schweregrad der Zirkulationsstörung und des Sauerstoffdefizits nach PCA kann mit hämodynamisehen Messungen nicht erfaßt werden. Dagegen erlaubt die Anwendung von Platin-Mehrdrahtelektroden die Untersuchung von Mikrozirkulationsstörungen anhand der Sauerstoffversorgung des Gewebes. An männlichen LewisRatten wurden in der normalen und zirrhotischen Leber Bestimmungen des Sauerstoffgehalts nach Anlage von 5 verschiedenen portosystemischen Anastomosen von der Organoberfläche aus durchgeführt. Portocavale End-zu-SeitShunts und portocavale Transpositionen resultierten in einem massiven Sauerstoffverlust der Leber, der nur bei normalem Lebergewebe kompensiert werden konnte. Durch einen Seit-zu-Seit-Shunt konnte eine wesentliche Verbesserung der O2-Versorgung der zirrhotischen Leber erreicht werden. Splenocavale Shunts und mesentericocavale Shunts konnten einen O2-Abfall nur teilweise kompensieren, verhinderten aber einen vollständigen O2-Entzug. Mit Hilfe der Pt-Elektrode konnte anhand der O2-Versorgung der Leber gezeigt werden, daß das der Leber zugeführte Restvolumen nicht entscheidend ist für die O2-Versorgung des Parenchyms, wenn nur die weitere Zufuhr von Pfortaderblut aus dem Pankreasbereich sichergestellt ist. Ein Zusammenwirken von hepatotrophen Faktoren und vasoaktiven Substanzen aus dem Gastrointestinaltrakt wird für möglich gehalten. Die klinische Anwendung dieser Mikrozirkulationsdiagnostik könnte einen erheblichen Einfluß gewinnen auf die Selektion der Shuntform bei Leberzirrhose und portaler Hypertension.
