Summary
Nine patients with impaired erection and a mean age of 41 years were diagnosed by means of a battery of multidisciplinary examinations: specific anamnesis, urological, neurological, endocrinological, angiological, psychosomatic and metabolic investigations, electromyography, measurement of the latency period and duration of the bulbocavernosus reflex, measurement of the nocturnal penis tumescence, penile blood-pressure index (PBPI), artificial erection with papaverine, flowmetry, phalloarteriography, dynamic cavernosonography. In all cases, an arterial and/or venous genesis was discovered as well as neurological or psychological factors. Three microsurgical techniques were employed: the end-to-side anastomosis of the A. epigastrica inferior preferably with the A. dorsalis penis (two cases), with the V. dorsalis penis profunda (four cases) and with the two combined (three cases). In the last cases, the vein was ligated or constricted proximal to the anastomosis. The primary factor in choosing the particular reconstruction method was the angiogram. Diagnosis entailed no complications for the patients. In addition to minor postoperative complications such as a hematoma, scar pains or hypervascularization of the glans and the corpus spongiosum, three early occlusions occurred in reconstructions up to one month postoperatively. One to five months after surgery the erectile potency had improved or normalized in seven out of nine patients, making intercourse possible. Two patients, the only two with a posttraumatic interruption in the bulbocavernosus reflex curve, showed no change. For purposes of postoperative control of arterio-venous anastomoses, the typical shunt sound is observed by stethoscope. After revascularization of the A. dorsalis penis, it is recommended that the PBPI be taken at the Aa. profundae penis and, when necessary, that a control angiography be performed. For the treatment of potency disturbances stemming from vascular causes, our results advocate that microsurgical vascular reconstruction be given due consideration before implantation of a penile prothesis.
Zusammenfassung
9 Patienten mit Erektionsstörung im Durchschnittsalter von 41 Jahren wurden mit Hilfe eines multidisziplinären Untersuchungs-programmes abgeklärt: spezifische Anamnese, urologische, neurologische, psychosomatische, endokrinologische, angiologische und Stoffwechseluntersuchung, Elektromyographie, Messung der Latenzzeit und Dauer des Bulbocavernosusreflexes, Messung der nächtlichen Penis-Tumescenzen, Penis-Blutdruck-Index, artefizielle Erektion mit Papaverin, Flowmetrie, Phallo-Arteriographie, dynamische Cavernosographie. In allen Fällen wurden eine arterielle und/oder venöse Genese, daneben aber auch neurologische oder psychische Komponenten aufgedeckt. Es kamen drei mikrochirurgische Operationsmethoden zur Anwendung: die End-zu-Seit-Anastomose der A. epigastrica inferior in erster Linie mit der A. dorsalis penis (2 Fälle) oder mit der V. dorsalis penis profunda (4 Fälle). Dreimal wurden beide Gefäße anastomosiert und die Vene proximal der Anastomose ligiert oder eingeengt. Für die Wahl der Rekonstruktionsmethoden war vor allem das angiographische Bild entscheidend. Die diagnostische Abklärung verlief ohne Zwischenfälle. Nach den operativen Eingriffen gab es neben geringfügigen Komplikationen wie einem Haematom, Narbenschmerzen oder Hypervascularisation der Glans und des Corpus spongiosum auch drei Frühverschlüsse von Rekonstruktionen nach spätestens 1 Monat. 1–5 Monate nach der Operation war die erektile Potenz bei 7 von 9 Patienten verbessert bis normalisiert, so daß ein Geschlechtsverkehr möglich war; bei 2 Patienten, den einzigen mit post-traumatischer Unterbrechung des Bulbocavernosusreflexbogens, war der Zustand unverändert. Für die postoperative Kontrolle arteriovenöser Anastomosen hat sich der auskultatorische Nachweis des typischen Shuntgeräusches bewährt. Nach Revascularisation der A. dorsalis penis ist entweder die Druckmessung an den Aa. penis profundae oder eine Kontroll-Angiographie zu empfehlen. Unsere Ergebnisse sprechen dafür, daß heute zur Behandlung von vasculär bedingten Potenzstörungen die mikrochirurgische Revascularisation vor der Implantation einer Penis-Prothese in Erwägung gezogen werden sollte.
