Summary
From 1972–1982 149 pathological fractures in 91 patients were treated. 134 of these were caused by malignant disease. With 78% metastases were the most common cause for fractures. 59% of the patients were women. The most frequent fracture localisation was the femur followed by ribs and humerus. 62% of the fractures but 100% of the leg fractures were treated operatively. The best functional results were achieved after compound osteosynthesis. The mean survival time of the patients was 10.7 months. The one year survival rate was 24.6% and the two year survival rate was 9.2%. The best survival rates were observed in women, patients with -breast cancer metastases, femur shaft- or subtrochanteric fractures. The lowest survival rate was found in male patients, patients with unknown primary tumour and femur neck fractures.
Zusammenfassung
Von 1972–1982 wurden 149 pathologische Frakturen bei 91 Patienten stationär behandelt. 134 dieser Frakturen traten bei malignen Grunderkrankungen auf. Metastasen stellten mit 78% die bei weitem häufigste Frakturursache. Das weibliche Geschlecht war mit 59% wesentlich häufiger betroffen. Das Femur, gefolgt von Rippen und Humerus stellten die häufigsten Frakturlokalisationen dar. 62% der pathologischen Frakturen und 100% der pathologischen Frakturen der unteren Extremität wurden operativ versorgt. Die funktionellen Ergebnisse waren nach der Verbundosteosynthese am günstigsten. Die mittlere überlebenszeit der Patienten mit maligner Grunderkrankung betrug 10,7 Monate. Die Einjahresüberlebensrate beträgt 24,6%, die Zweijahresüberlebensrate 9,2%. Weibliche Patienten, Patientinnen mit Mammacarcinommetastasen sowie Patienten mit Frakturlokalisation im Femurschaft und im subtrochanteren Bereich weisen eine überdurchschnittlich günstige Prognose auf. Männer, Patienten mit pathologischen Schenkelhalsfrakturen und unbekanntem Primärtumor wiesen die schlechteste Prognose auf.
