Summary
The injurious effect of parenterally administered Thorotrast is due to storage of the compound by cells of the RES. This stimulates proliferation and radiation damage to the storing and the surrounding cells. Cold and hot lymphography has similar effects. Intake of the oily contrast medium causes foreign body reactions and increasing fibrous obliteration of lymph nodes. The added I131 produces radiation damage to the surrounding tissue. The question is discussed wether the113I-labelled oily contrast medium, which after hot lymphography enters the lungs through the thoracic duct, is likely to trigger off radiation pneumonitis with subsequent fibrosis or, at least, will favour its development when the mediastinum is irradiated percutaneously at the same time.
Zusammenfassung
Die schädigende Wirkung parenteral verabreichten Thorothrastes beruht auf der Speicherung der Substanz durch Zellen des RES mit entsprechenden Proliferationsreiz und gleichzeitiger Strahlenschädigung der speichernden und der umgebenden Zellen. Ähnliche Auswirkungen hat die kalte und heiße Lymphographie. Die Aufnahme des öligen Kontrastmittels führt zu Fremdkörperreaktionen und zunehmender bindegewebiger Verödung der Lymphknoten, das beigegebene J131 zur Strahlenschädigung des umgebenden Gewebes. Diskutiert wird die Frage, ob das nach heißer Lymphographie über den Duetus thoracicus in die Lunge gelangende J131-markierte ölige Kontrastmittel die Entstehung einer Strahlenpneumonitis mit anschließender Strahlenfibrose auslösen oder bei gleichzeitiger pereutaner Bestrahlung des Mediastinums begünstigen kann.
