Summary

The clinical importance of perfusion, inhalation, and blood space scintigraphy and the determination of precordial activity in the diagnosis of surgical disorders of the thoracic organs is delineated by means of typical case reports. Disorders of circulation subsequent to pulmonary embolism constitute the chief indication for perfusion scintigraphy, which is much superior to x-ray diagnosis. The early diagnosis of central carcinoma of the bronchus and the differential diagnosis of expanding and infiltrating central and mediastinal processes may be improved by means of perfusion scintigraphy. In benign processes local blood flow may be assessed preoperatively with greater exactitude. Measurements of precordial activity enables left-right-shunt formation to be shown. Blood space scintigraphy improves the differentiation of paracardial and paramediastinal opacities in the x-ray picture.

Zusammenfassung

Anhand typischer Befunde wird die klinische Bedeutung der Perfusions-, Inhalations- und Blutraumszintigraphie sowie der präkordialen Aktivitätsmessung in der Diagnostik chirurgischer Erkrankungen der Thoraxorgane definiert. Die Hauptindikation zur Perfusionsszintigraphie liegt bei den Zirkulationsstörungen als Folge der Lungenembolie, bei der sich eine Überlegenheit gegenüber der Röntgendiagnostik zeigt. Die Frühdiagnostik des zentralen Bronchialcarcinoms und die Differentialdiagnostik expansiv und infiltrativ wachsender zentraler und mediastinaler Prozesse kann mit Hilfe der Perfusionsszintigraphie verbessert werden. Bei gutartigen Prozessen kann die lokale Durchblutung präoperativ exakter beurteilt werden. Die präkordiale Aktivitätsmessung ermöglicht den Nachweis einer Links-Rechts-Shuntbildung. Die Blutraumszintigraphie erlaubt eine bessere Differenzierung röntgenologisch nachgewiesener parakardialer und paramediastinaler Verschattungen.
