4. Fortschritte in der Diagnostik chirurgischer Krankheitsbilder durch Lymphographie

Summary

In the diagnosis of surgical diseases lymphangioadenography is important for the demonstration of metastases in the lymph-nodes, for assessing the state of spread of primary lymph-node neoplasms, and for the elucidation of lymphangiopathies.

Lymphography is indicated when searching for metastases of all tumours of testicles and penis and for gynaecological carcinomas. It is also indicated for malignant tumours of the extremities (melanomas and sarcomas) as well as for all primary retroperitoneal tumours. According to previous experience there is only a relative indication when looking for metastases from carcinomas of the bladder, colon and breast.

In all patients with primary lymph node neoplasias retroperitoneal manifestation should be especially excluded by lymphography. Finally, lymphography elucidates the various types of primary and secondary lymph oedemas as well as the cause of chylous reflux and chylous extravasation.

Zusammenfassung

In der Diagnostik chirurgischer Krankheitsbilder hat die Lymphangioadenographie Bedeutung im Nachweis von Lymphknotenmetastasen, bei der Feststellung des Ausbreitungsstadiums primärer Lymphknotenneoplasien sowie bei der Klärung der Lymphangiopathien.

Es besteht eine absolute Indikation für die Lymphographie bei der Metastasensuche aller Testis- und Penistumoren sowie für alle gynäkologischen Carcinome. Weiterhin für die malignen Tumoren der Extremitäten (Melanome und Sarkome) sowie für alle primären retroperitonealen Tumoren. Nach den bisherigen Erfahrungen besteht nur eine relative Indikation bei der Metastasensuche der Harnblasen-, Colon- und Mammacarcinome.

Bei allen primären Lymphknotenneoplasien sollte grundsätzlich durch eine Lymphographie eine retroperitoneale Manifestation ausgeschlossen werden. Schließlich klärt die Lymphographie die verschiedenen Formen der primären und sekundären Lymphödeme sowie die Ursache der chylösen Ergüsse.
