Summary
It was confirmed by 114 animal experiments that massive fat embolism of the lungs (and perhaps also of the kidneys) is chiefly caused by fat absorption in the injured limb. Lipolysis, absorption, and the manifestations of fat embolism are intensified by hypovolaemia and by partially compensated acidosis, whilst in primary alkalosis any fat embolism that might occur is only slight.
The acidotic rise of pulmonary arterial pressure is responsible for the opening up of the arterao-venous anastomoses of the lungs and for the inflow of chylomicrons into the arterao-vascular limb. When pH falls below 7.2, the clearance and phagocytosis (RES) of plasma neutral fats is delayed and this may reduce the activity of lipase.
Early and sufficient substitution of volume alone by solutions of electrolytes and plasma substitutes (dextran 40, hydroxyethylene starch) is not a sufficient safeguard against manifestations of fat embolism. Only additional i.v. administration of essential phospholipids (Lipostabil) and/or fibrinolytic (heparin) and antiacidotic compounds (THAM, or better NaHCO3.) will provide sufficient protection against fat embolism in the initial stages.
Zusammenfassung
Bei 114 tierexperimentellen Untersuchungen findet sich bestätigt, daß eine massive Fettembolie der Lungen und evtl. der Nieren vorwiegend durch Fettresorption im Bereich der traumatisierten Extremität auftritt. Lipolyse, Resorption und Manifestation der Fettembolie werden durch Hypovolämie und teilweise kompensierte Acidose intensiviert, während unter primärer Alkalose nur eine mäßige Fettembolie auftritt. Die acidotische Pulmonalarteriendrucksteigerung wird für die Eröffnung der arterio-venösen Anastomosen der Lunge und die Einschwemmung von Chylomikronen in den arteriellen Gefäßschenkel verantwortlich gemacht. Bei pH-Abfall unter 7,2 wird die Clearance und Phagocytose (RES) der Plasmaneutralfette verzögert und unter Umständen die Lipaseaktivität herabgesetzt. Eine frühzeitige und ausreichende Volumensubstitution mit Elektrolyt- und Plasmaersatzlösungen (Dextran 40, Hydroxyäthylenstärke) ist allein keine ausreichende Gewähr gegenüber der Manifestation einer Fettembolie. Erst die zusätz liche i.v. Applikation von essentiellen Phospholipiden (Lipostabil) und/oder fibrinolytischen (Heparin) sowie antiacidotischen Substanzen (TRAM, besser NaHCO3) bewirkt eine genügende Prophylaxe gegenüber der Fettembolie im Initialstadium.
