Summary
Visualisation of the coronary arteries with a contrast medium is necessary for surgical procedures aiming at improving myocardial profusion. According to our coronariographic finding in more than 150 patients with coronary artery disease, an incomplete or complete, circumscribed coronary artery occlusion which is located near the aorta is to be expected in almost 10% of the cases. This coronary artery occlusion can be eliminated by a direct reconstructive procedure, for example endarterectomy and vein patch. This is also valid for thrombotic coronary artery occlusion due to trauma. After blunt thoracic trauma one should take this into consideration more frequently. In patients with generalized coronary sclerosis, however, only conservative or indirect surgical measures (e.g. Vineberg's implantation of the internal mammary artery and its modification) can be considered. On the basis of our vasographic control examinations after long-term oral treatment with specific coronary drugs one must also expect the development of a pharmacologically induced collateral circulation in human beings. The potential extracardiac collateral anastomoses to the coronary arteries must be preserved during thoracic procedures.
Zusammenfassung
Operative Eingriffe zur Verbesserung der myokardialen Durchblutung setzen eine Kontrastmitteldarstellung der Coronararterien voraus. Nach unseren coronarographischen Befunden bei über 150 coronarkranken Patienten ist in nahezu 10% ein umschriebener, aortennahe gelegener inkompletter oder kompletter Coronararterienverschluß zu erwarten, der durch einen direkten rekonstruktiven Eingriff, z. B. Endarterektomie und Venenpatch, beseitigt werden kann. Dasselbe gilt auch für den traumatisch bedingten thrombotischen Coronararterienverschluß, an den öfters nach einem stumpfen Thoraxtrauma gedacht werden soll. Bei der generalisierten Coronarsklerose kommen dagegen nur konservative oder indirekte operative Maßnahmen (z. B. die Vinebergsche Mammaria interna-Implantation und ihre Modifikationen) in Frage. Aufgrund unserer vasographischen Kontrolluntersuchungen nach peroraler Langzeitbehandlung mit coronarspezifischen Mitteln muß auch beim Menschen mit der Entwicklung eines pharmakologisch induzierten Kollateralkreislaufes gerechnet werden. Die potentiellen extrakardialen Kollateralverbindungen zu den Coronararterien müssen bei thorakalen Eingriffen geschont werden.
