14. Diagnostische Maßnahmen bei coronaren Herzerkrankungen

Summary

After a brief description of diagnostic routine measures the author mainly refers to the importance of coronary arteriography for the assessment of coronary cardiac diseases. The accuracy of this technique - demonstrated with a film - is good, since it permits visualisation of vessels up to a magnitude of 0.1 to 0.2 mm.

In addition to coronary arteriography preoperative determination of ventricular function with the aid of left ventricolography as well as enddiastolic left ventricular-pressure, minute volume and certain metabolic parameters of the myocardium at rest and during stress are of decisive importance. The author discusses the indication for reconstructive surgical procedures on the coronary arteries and revascularisation procedures.

Zusammenfassung

Nach einer Schilderung diagnostischer Routinemaßnahmen wird in erster Linie auf die Bedeutung der Coronararteriographie für die Beurteilung der coronaren Herzerkrankungen eingegangen. Die Leistungsfähigkeit dieser Technik - an Hand eines Filmes demonstriert - ist gut, da sie eine Darstellung von Gefäßen bis zu einer Größenordnung von 0,1-0,2 mm erlaubt.

Neben der Coronararteriographie sind die preoperative Erfassung der Ventrikelfunktion mit Hilfe der Linksventrikolographie sowie von enddiastolischem linken Ventrikeldruck, Minutenvolumen und bestimmten Myokardstoffwechselgrößen in Ruhe und während Belastung von entscheidender Bedeutung. Die Indikationen für reconstruktive Coronaroperationen und Revascularisationsmaßnahmen werden diskutiert.
