Summary
Lymphatic effusions are classified into traumatic, spontaneous or “idiopathic”. Definite differentiation from other types of fluid accumulations can only be achieved with the aid of laboratory, chemical and microscopical methods. In appr. 1/3 to 1/2 of all cases it is possible to demonstrate that trauma is responsible for the chylothorax. 0.018 to 0.5% of intrathoracic procedures are associated with iatrogenic injuries to the thoracic lymphatic vessels. Congenital chylothorax and isolated chylopericardium are very rarely observed. Conservative treatment consists in decompression aspiration, special diet (MCT) and negative pressure drainage. If this fails, surgical ligation of the thoracic duct proximal to the fistula is the best method. Spontaneous lymphatic ascites is caused by malignant tumors and hepatic cirrhosis, in children it is caused by congenital malformations of the lymphatic vascular system. Hitherto surgical treatment did not produce satisfactory results; on account of this symptomatic conservative treatment with aspirations and special diet (MCT) is to be preferred.
Zusammenfassung
Man unterscheidet traumatisch, spontan oder „idiopathisch” entstandene chylöse Ergüsse. Die sichere Abgrenzung gegen anders geartete Flüssigkeitsansammlungen gelingt nur mit Hilfe laborchemischer und mikroskopischer Methoden. In etwa 1/3–1/2 aller Fälle läßt sich der Chylothorax auf ein Trauma zurückführen. Intrathorakale Eingriffe sind in 0,018–0,5% mit iatrogenen Verletzungen des Brustlymphganges belastet. Der kongenitale Chylothorax und das isolierte Chyloperikard werden äußerst selten beobachtet. Die konservative Therapie besteht in Entlastungspunktionen, Spezialdiät (AICT) und Saugdrainage. Bei ihrem Versagen ist die operative Unterbindung des D. thorac. proximal der Fistel das beste Verfahren. Der spontane Chylascites wird durch bösartige Tumoren und Lebercirrhose, bei Kindern durch angeborene Fehlbildungen des Lymphgefäßsystems verursacht. Die operative Behandlung brachte bisher keine befriedigenden Ergebnisse; daher ist die symptomatische konservative Therapie mit Punktionen und Spezialdiät (MCT) vorzuziehen.
