Summary
Even the present time surgical treatment of lymphedema does not produce entirely satisfactory results. The older methods such als lymphangioplasties and myolymphangioplasties have failed completely. The following remained: thecondoleonic operation which has beenextended according to Sistrunk. It may be adequate for cases of medium severity but it is inadequate for cases with very pronounced elephantiasis.
In such cases one can consider: thesuperficial total lymphangiectomy according to Servelle. It consists of excision, as complete as possible, of the adematose subcutaneous cellular tissue together with the underlying fascia. Servelle has used this operation in more than400 cases with good long-term results.Charles' operation which is widely used in England, differs from the surgical procedure according to Servelle only in so far as the total skin is removed at the same time and replaced by a graft. After this operation necrosis of the graft and verucous changes are not infrequent and they have an unpleasant cosmetic effect. Thus, at the present time, the best solution of this therapeutic problem appears to belymphvessel transplantation by means o f a shaved skin flap according to the technique o f the Englishman Thompson. This technique is described and assessed in detail. In contrast to this,lymphatic-venous anastomosis according to Nielubowicz, which is theoretically very interesting, was found of little use in actual practice. Late control of 28 cases which were operated according to various methods confirm the previously mentioned final conclusions.
Zusammenfassung
Durch die operative Behandlung des Lymphödems erzielt man auch heute noch nicht durchwegs zufriedenstellende Ergebnisse. Die älteren Methoden, wie die Lymphangioplastiken und die Myolymphangioplastiken haben so ziemlich ganz versagt. Übrig bleiben: die nachSistrunk erweiterte Kondoleonische Operation. Sie kann für mittelschwere Formen genügen, reicht aber bei sehr stark ausgeprägter Elephantiasis nicht aus.
In Frage kommen dann: dieoberflächliche totale Lymphangiektomie nach Servelle. Sie besteht in einer möglichst vollständigen Excision des ödematösen Unterhautzellgewebes zusammen mit der darunterliegenden Fascie. Servelle hat diese Operation in mehr als 400 Fällen mit guten Spätergebnissen ausgeführt. Die in England viel geübteCharles Operation unterscheidet sich von dem Servelleschen Eingriff nur dadurch, daß gleichzeitig die ganze Haut mitentfernt und durch eine Transplantation ersetzt wird. Nekrosen des Transplantates und veriköse Entartungen sind nach dieser Operation nicht selten und wirken unschön.Die Lymphgefäßtransplantation mittels eines abrasierten Hautlappens nach der Technik des Engländers Thompson, die eingehend beschrieben und bewertet wird, erscheint deshalb den Referenten als die augenblicklich beste Lösung des therapeutischen Problems. Dagegen hat sich dielympho-venöse Anastomase nach Nielubowicz, theoretisch sehr interessant, praktisch nur wenig bewährt. Die S:atkontrolle von 28 nach verschiedenen Methoden operierten eigenen Fällen bestätigt die oben erwähnten Schlußfolgerungen.
