Summary
Postoperative rupture of the abdominal wall is defined as the asymptomatic or minimally symptomatic drawing apart of the layers of the abdominal wall without a local inflammatory reaction of the wound margins and without primary infection. Hitherto the actual histological cause has not been clarified. Questionable and definitely favourable factors are discussed by the author. Disorders of the process of wound healing form the main aspect. The frequency in our patient material consisting of 7503 cases was 1.050/0. Malignant lesions were predominant and accounted for 55.70/0 of the cases. Hitherto the etiology of abdominal incisional hernias has also not been definitely determined. The author discusses the following factors: wound infections, a point of least resistance, the direction of the incision, antibiotic agents, increased postoperative fibrinolysis and others. The indication for surgery — particularly in patients with large incisional hernias with a relative indication — must be made in a critical manner. As far as surgical care is concerned, autogenous tissue was found to be definitely superior to plastics. Direct approximation of the margins of the hernia was preferred in 207 cases which were operated upon by the author. Free plastics were only carried out in 6 cases. The recurrence rate was 130/0.
Zusammenfassung
Die postoperative Bauchwandruptur wird definiert als das symptomlose oder symptomarme Auseinanderweichen der Bauchwandschichten ohne lokale entzündliche Reaktion der Wundränder und ohne primäre Infektion. Die eigentliche Entstehungsursache ist noch nicht geklärt. Fraglich und sicher begünstigende Faktoren werden zur Diskussion gestellt. Störungen des Wundheilungsgeschehens stehen im Vordergrund. Die Häufigkeit beim eigenen Krankengut von 7503 Fällen betrug 1,050/0. An erster Stelle stand das Malignom mit 55,70/0. Auch die Atiologie des Bauehnarbenbruches ist noch nicht sicher bekannt. Als Faktoren diskutiert werden die Wundinfektion, ein Locus minoris resistentiae, Schnittführung, Antibiotics, eine gesteigerte postoperative Fibrinolyse u. a. Die Op.-Indikation — vor allem bei großen Narbenbrüchen mit relativer Indikation —ist kritisch zu stellen. Bei der operativen Versorgung hat das körpereigene Gewebe vor den Kunststoffen eindeutig den Vorrang erzielt. Bei eigenen 207 operierten Fällen wurde die direkte Vereinigung der Bruchränder bevorzugt. Freie Plastiken wurden nur in 6 Fällen durchgeführt. Die Rezidivquote betrug 130/0.
