Lymphographie beim Rectumcarcinom

Lymphography in cancer of the rectum

Summary

Cancer of the rectum is participated in 0.72% in the entire section material of the Department of Pathology, Göttingen during the period 1942 to 1968. Of 178 patients with cancer of the rectum 71 (39,9%) showed evidence of metastatic disease. Metastases in the lymphatic system were predominantly situated in the paraaortic and iliac region. The most frequent haematogen metastases could be found in the liver. In correlation to the state of primary tumor metastases become more frequent in advanced cases. In patients having died short after operation (without any sign of tumor) at section in 18,1% and in those with cancer growth beyond the rectum in 60,5% metastases can be demonstrated. Among 70 patients being investigated lymphographically a positive diagnosis in the pelvis in 38.7% and in 17.2% in the lumboaortic region could be made. Most of metastases could be showed in patients with inoperable primary tumor.

Lymphography should be performed 6 to 8 days before a planned operation.

Zusammenfassung

Das Rectumcarcinom macht 0,72% aller Sektionen des Path. Inst. Göttingen im Zeitraum von 1942 bis 1968 aus. Unter den 178 sezierten Rectumcarcinomen zeigen 71 (39,9%) Metastasen. Von den lymphogenen Metastasen sind die paraaortalen und iliacalen am häufigsten. Von der hämatogenen Metastasierung steht die in der Leber im Vordergrund. Bezogen auf den Lokalbefund läßt sich im Sektionsgut eine signifikante Bevorzugung der Metastasierung bei fortgeschrittenen Stadien feststellen. Bei den postoperativ Verstorbenen (ohne Rest-Tu.) kommt eine Metastasierung nur in 18,1%, bei den mit einem Tumorwachstum über das Rectum hinaus in 60,5% vor. Unter 70 lymphographisch untersuchten Rectumcarcinomen läßt sich in 38,7% ein positiver Befund im Beckenund in 17,2% im Lumbalbereich feststellen. Die lymphographisch festgestellte Metastasierung ist am häufigsten bei den lokal inoperablen; metastasierenden Rectumcarcinomen.
