Summary
The prevention of duodenogastric reflux by a jejunal transposition was studied in 11 patients following Billroth 11 to Billroth I reinterventions. For this purpose intragastric lysolecithin and bile acids were measured. While resected stomachs with a 25 cm isoperistaltic jejunal transposition showed normal reflux values, shorter jejunal segments still had a pathological reflux. The role of the jejunal transposition as an antireflux procedure in ulcer surgery is discussed.
Zusammenfassung
An 11 Patienten mit einer Billroth II - Billroth 1 Umwandlungsoperation mit isoperistaltischer Jejunuminterposition wurde der Refluxschutz durch die Interponate untersucht. An Refluxparametern dienten Gallensäuren und Lysolecithin. Nach ausreichend langer (25 cm) Jejunuminterposition normalisierten sich die Refluxwerte des Resektionsmagens. Kürzere Interponate ließen noch pathologische Werte erkennen. Die Bedeutung der Jejunuminterposition als taktisches Prinzip in der Ulcuschirurgie wird diskutiert und auch ihren besonderen Stellenwert im Rahmen der Refluxkrankheit des Magens hingewiesen.
