Summary
Intraabdominal heterotopic duodenopancreatic allotransplantations into the right iliac fossa were performed in 81 pancreatectomized dogs. Varying anastomoses were done in the different experimental groups. The end-to-side anastomosis between the portal vein of the transplant and the inferior cava vein of the recipient was superior to the end-to-end anastomosis with the right common iliac vein of the recipient, which often led to thromboses and pancreatitis. The end-to-end anastomosis to the right external iliac artery of the recipient showed better results than that to the right renal artery of the recipient.
Without immunosuppressive therapy acute rejections with terminal hyperglycemia and hyperdiastasemia were observed. The survival times of the recipient dogs were markedly prolonged by daily administration of 4mgm/kgm Azathioprine and 2 mgm/kgm Prednisolone. The grafts showed signs of chronical rejection. In most instances the endocrine function of the pancreatic grafts was maintained until the recipients died. The difficulty to early recognize rejection crises in immunosuppressed recipients of pancreatic allografts is emphasized.
In 6 of 12 cases the endocrine pancreas function was maintained to a certain degree, when the grafts were preserved by freezing at −60 °C for 6 to 10 days and then transplanted. It is supposed, that toxic substances are released from the preserved grafts.
The problems of preservation of whole organs by freezing as well as the applicability of pancreatic transplantation in humans are discussed.
Zusammenfassung
An 81 pankreatektomierten Hunden wurden intraabdominelle heterotope Duodenopankreas-Allotransplantationen in die rechte Fossa iliaca ausgeführt, wobei die Art der Gefäßanastomosen in den verschiedenen Gruppen variierte. Die End-zu-Seit-Anastomose zwischen der V. portae des Transplantates und der V. cava inferior des Empfängers ist der End-zu-End-Anastomose mit der V. ihaca communis dextra vorzuziehen, da im letzteren Fall gehäuft venöse Thrombosen und Pankreatitiden auftraten. Die End-zu-End-Anastomose mit der rechten A. iliaca externa des Empfängers ist einer solchen mit der rechten A. renalis deutlich überlegen.
Ohne immunosuppressive Therapie traten akute Abstoßungen mit terminaler Hyperglykämie und Erhöhung der Serumdiastase ein. Durch 4 mg/kg Azathioprin und 2 mg/kg Prednisolon täglich wurden die Überlebenszeiten der Empfänger deutlich verlängert. Die Transplantate zeigten chronische Abstoßungsreaktionen. Die endokrine Funktion der Transplantate war meist bis zum Exitus erhalten geblieben. Auf die Schwierigkeit der Früherkennung von Abstoßungskrisen bei immunosupprimierten Empfängern wird hingewiesen. Nach Verpflanzung von Transplantaten, die für 6 bis 10 Tage bei −60°C gefrierkonserviert worden waren, zeigte sich, daß in 6 von 12 Fällen die endokrine Pankreasfunktion bis zu einem gewissen Grad erhalten geblieben war. Es wird die Freisetzung toxischer Substanzen sus den konservierten Organen angenommen. Die Problematik der Gefrierkonservierung von Organen und die klinische Anwendbarkeit der Pankreastransplantation werden diskutiert.
