The clinical relevance of intraoperative electrophysiologic mapping

Summary

Electrophysiological intraoperative mapping is described as a technique for identification of the specialized AV conduction system during open heart surgery; furthermore it is suitable for delineating the morphological origin of ventricular tachycardias which require surgical therapy. The mapping technique is described with respect to its methodical aspects and illustrated by means of congenital and acquired heart disease.

Zusammenfassung

Das intraoperative Mapping ist eine angewandt-elektrophysiologische Methode, mit welcher sich während des kardiochirurgischen Eingriffs Verlauf des Hisschen Bündels und Ursprungsort tachykarder Herzrhythmusstörungen lokalisieren lassen. Die klinische Bedeutung dieser Lokalisationsdiagnostik liegt darin, daß in Kenntnis der Topographie dieser elektrophysiologisch relevanten Strukturen unter Umständen chirurgische Verletzungen der spezifischen Muskulatur vermieden und konservativ-therapieresistente Tachykardien einer operativen Therapie zugeführt werden können. Die Methode wird in ihrer Technik beschrieben und anhand kasuistischer Beispiele bezüglich ihres klinischen Wertes erläutert.
